Hospice Clinical Educator (LVN or RN)

Location: Must be located in CA and willing to travel to branch locations throughout the US at least once a month
The Clinical Educator is responsible for maintaining and implementing education to all clinical staff. Works closely with all Branch Director and Field staff supervisor (FSS) to monitor educational standards. Will monitor documentation, clinical skills, set up workshops for ongoing education. Will work with FSS on the clinical orientation processes. Facilitates consistency of standardized education policies, procedures and programs. Partners with Clinical Operations, Performance Improvement, Compliance and Service Operations to ensure education and training is aligned with current clinical standards, guidelines, relevant statutes, rules and regulations.
- Develops clinical competencies.
- Develops, implements and evaluates the orientation program for new organization personnel, and training programs to expand the capacity of all staff through in-services and continuing education programs.
- Focus on PDGM (Patient Driven Groupings Model) and HVVBP education.
- Implements practices Identified to improve clinical delivery, quality and service.
- Assesses the learning needs and competence of staff to improve the level of performance, teaching skills, and quality of care, and assure compliance with accreditation standards
- Develop curriculum and ability to precept.
- Trends and analyzes utilization, outcomes, and quality of program data on an ongoing basis.
- Works closely with all Branch Directors, Rehab Director and Quality Care Mangers to monitor educational standards and deficits.
- Reviews clinical documentation and skills for ongoing learning needs.
- Ensures new hire education and ongoing staff training is aligned with current clinical standards, guidelines, relevant statutes, rules, and regulations.
- Provides education and support to the clinical staff in maintaining quality improvement outcomes and STAR ratings.
- Participates in the development, implementation, and evaluation of best practices for Home Care/Hospice.
- Assists Clinical Managers in evaluating the performance of new clinicians.
- Regularly evaluates the appropriateness of the educational courses assigned to staff.
- Facilitates learning experiences for staff through direct and indirect methods.
- Ability to work and communicate via electronic communications, and possess knowledge of HCHB, SHP, Microsoft Word, Excel, and PowerPoint
- Licensed / Registered Professional in the state of California.
- Associate degree or higher from an accredited institution.
- Recent clinical experience within Hospice
- Working knowledge of HCHB (EMR)
- Demonstrated ability to work effectively in a team.
- Excellent observation, verbal, and written communication skills.
- Knowledge of State and Federal regulations in Hospice.
