We are looking for an experienced, talented, and compassionate Hospice Admissions RN for a full-time position located in the South Shore area.
Available Schedule: Sunday, Monday, Tuesday 11am-11pm or Sunday, Monday, Wednesday 11am - 11pm
What will you be doing:
The Hospice Admissions RN is responsible for conducting the initial in-home evaluation of the patient and development of the care plan under the direction of the MD. Strong assessment skills are a must and the ability to identify a patient’s needs at home and make referrals to other disciplines. You must also communicate effectively with the ordering provider when conducting the initial assessment to ensure accuracy of the care plan and all medications.
- Develop, implement, and evaluate patient care plans; involve family/patient and complete necessary documentation per policies.
- Conduct initial start-of-care visits; initiate, develop, and modify the care plan as needed, addressing or documenting rationale for unaddressed issues.
- Implement diagnostic, preventive, and rehabilitative procedures; refer to other disciplines as needed or document rationale for not doing so.
- Assess and monitor patient care needs; inform physicians and other health professionals of any changes in condition.
- Teach, supervise, and counsel patients/families on home care procedures and needs; ensure infection control and safety measures are in place.
- Supervise home health aide services per agency policy; assess and revise care plans as needed, reporting changes to the Clinical Manager.
- Coordinate care plans and collaborate with other health professionals; attend/initiate client care conferences for complex cases.
- Maintain accurate, timely, and legible records and reports.
- Participate in continuing education and in-service activities to enhance professional skills and growth.
What we’re looking for in you:
- Current Massachusetts Nursing license in good standing.
- Registered Nurse (Bachelor's degree preferred).
- 2+ years of nursing experience required.
- Hospice experience strongly preferred.
- Strong clinical assessment skills and the ability to prioritize and triage patients.
- Exceptional communication skills and the ability to work within a team structure.
- Excellent organizational skills and time management skills.
- Ability to work independently and understanding of how to access resources for patients.
- Strong computer skills, experience in Oasis.
