Optimal Care, Inc. is seeking an Occupational Therapist committed to providing the highest quality care in our service area.
We offer competitive visit rates and excellent working conditions. Home care experience is preferred, but not required.
Occupational Therapist Responsibilities:
- Provides skilled occupational therapy services in accordance with the physician's plan of care.
- Initial evaluation and ongoing assessments of the client's level of functioning including OASIS assessments at appropriate time points by applying diagnostic and prognostic procedures and tests.
- Develops and revises the plan of care in consultation with the physician and other care team members and sets goals for patients as appropriate.
- Treats the client through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function.
- Observes, records, and reports the clients reaction to treatment and any changes in the client's condition to the physician and/or care manager, case manager, and other care team members in a timely manner. Evaluates outcomes of care.
- Teaches, supervises, and counsels the client and family regarding home rehabilitation procedures and other care needs as appropriate to the client's condition. Utilizes agency educational material as appropriate.
- Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager.
- Demonstrates and teaches alternate techniques to complete activities of daily living, proper transfers, and positioning.
- Orders and/or fabricates orthotic/adaptive/assistive devices and durable medical equipment.
- Evaluates, and regularly re-evaluates, the care and equipment needs of the client; develops, implements and revises the client's plan of care. Supervises, instructs, and advises certified occupational therapy assistant in patient-specific plan of care. Ensures that the plan of care addresses all problems identified in the assessment.
- Initiates diagnostic, preventative, and rehabilitative procedures as appropriate to the client's care and safety and makes referrals to other disciplines as indicated by the needs of the client under the direction of the physician.
- Keeps abreast of occupational therapy trends and knowledge.
- Participates in in-service programs.

Requirements:
- Graduate from an accredited Occupational Therapy program
- Occupational Therapy license recognized by the State of Michigan
- Minimum of one (1) year experience as a licensed therapist
- Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
- Demonstrates knowledge of the principles of growth and development over the life span
- Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
- Demonstrates computer skills sufficient to use the electronic medical record effectively
