Now Hiring: Full Time Licensed Practical Nurse
Territory: Lansing
Optimal Care is seeking a Full Time Licensed Practical Nurse committed to providing the highest quality of care.
We are looking for highly professional, self-motivated LPNs who love working with the adult and geriatric homebound population. Home care experience is preferred but not required.

Licensed Practical Nurse Responsibilities:
- Provides direct patient care as defined in the State of Nursing Practice Act
- Implements plan of care indicated by the clinician
- Provides accurate and timely documentation consistent with the plan of care
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Participates in coordination of home care services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, SLP) to the case manager or Clinical Supervisor
- Uses equipment and supplies effectively and efficiently
- Participates in personal and professional growth and development
- Performs other duties as assigned by the case manager or supervisor
Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program.
- Currently licensed by the state in which practicing
- Minimum of 1 year experience working as a LPN preferred
- Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
- Demonstrates knowledge of the principles of growth and development over the life span
- Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
- Demonstrates computer skills sufficient to use the electronic medical record effectively
- Valid Driver's License
