Job Summary:
The Director of Value-Based Care is responsible for leading the strategy, execution, and optimization of programs that enhance patient outcomes, reduce total cost of care, and maximize performance across value-based contracts. This role oversees three core divisions - Risk Stratification, Quality Gap Closure, and Population Health Management - ensuring seamless integration of clinical operations, analytics, and contract strategy.
The Director will be accountable for driving strategic alignment, operational success, and performance outcomes across assigned populations and value-based arrangements, advancing both organizational quality objectives and financial performance.
Responsibilities:

Lead and execute the organization’s value-based care strategy across all risk contracts and population health initiatives.

Ensure all value-based care programs meet financial performance targets and contribute to organizational sustainability.

Effectively communicate ongoing performance with the Chief Medical Officer, CEO, and executive leadership team to educate and drive clinical quality performance.

Oversee high risk patient management, transitions of care, and coordination processes to enhance patient outcomes and ensure smooth transitions across care settings.

Ensure seamless integration of clinical quality operations, data analytics, and technology to support program performance.

Monitor and evaluate program performance, implementing adjustments as needed to target financial performance and support evolving program/payor requirements.

Drive value-based membership growth and attribution management through collaboration with operations, marketing, business development, and payors.

Build and maintain strong relationships with external partners, including payors, specialist groups/providers, and networks, ensuring alignment with value-based care objectives.

Lead, mentor, and develop a high-performing value-based care team, promoting professional growth and fostering a collaborative, inclusive, and accountable culture.

Maintain fluency in all value-based contracts, program/payor portals, HEDIS quality measures, HCC and risk adjustment protocols, and incentive payment methodologies.
Qualifications

Bachelor's degree in healthcare administration, business administration, or related field; Master's degree preferred.

Minimum of 7-10 years of progressive leadership experience in healthcare management, preferably within a value-based care or accountable care organization.

Proven track record of successfully implementing and managing Population Management and Value-Based Care programs, ideally in a leadership role working with physician practices.

Strong leadership skills with the ability to influence and engage stakeholders at all levels.

In-depth knowledge of healthcare regulations, reimbursement models, and industry trends.

Prior experience handling a P&L or accountability for a large-scale project, initiative, or department.

Experience working with electronic medical records, healthcare data, and reporting.
