We are seeking a Care Coordinator to join our Care Delivery Team in a fast-paced, high-volume inbound support role. This position serves as a primary point of contact for patients, managing inbound communications via phone, email, chat, and SMS text message. As a key liaison between patients and their multidisciplinary care team, you will deliver a seamless, compassionate, and efficient experience by addressing questions related to scheduling, billing and insurance, pharmacy needs, referrals, clinical inquiries, and general program support. What You’ll Do

  • Own high-volume inbound patient and provider communication queues across phone, email, chat, and SMS, responding quickly and efficiently while maintaining accuracy and professionalism.
  • Consistently meet or exceed productivity, responsiveness, and quality metrics in a fast-paced environment.
  • Serve as the first point of contact for patients, resolving inquiries in real time without unnecessary handoffs.
  • Provide clear, confident guidance on general questions about the Oshi program, services, and care model.
  • Schedule, reschedule, and coordinate appointments quickly and accurately, ensuring minimal delays and clean handoffs to GI Providers, Registered Dietitians, and Behavioral Health Providers.
  • Respond to high-level billing and insurance questions, clearly explaining benefit design and cost-share in a way patients understand.
  • Triage clinical questions accurately and efficiently, ensuring providers receive complete, organized information.
  • Act as a reliable liaison between patients and their care teams, driving timely follow-up and closing communication loops.
  • Manage pharmacy-related inquiries, prescription coordination, and refill requests with urgency and attention to detail.
  • Identify urgent or complex concerns quickly and escalate appropriately, exercising sound judgment.
  • De-escalate challenging interactions with confidence, professionalism, and empathy.
  • Document all patient interactions in real time, maintaining thorough and accurate records.
  • Proactively identify workflow inefficiencies and suggest improvements that increase speed and patient satisfaction.
  • Cross-train and flex into additional responsibilities as business needs evolve.

Who You Are

  • 3+ years of experience in  remote patient scheduling, healthcare support or a high-volume call center environment (telehealth or digital health strongly preferred).
  • Proven ability to manage heavy inbound volume while maintaining speed, quality, and composure.
  • A self-starter who works independently, takes initiative, and requires minimal supervision.
  • Highly organized and detail-oriented, with the ability to multitask and clear queues efficiently.
  • Fluent in healthcare terminology and comfortable navigating clinical concepts.
  • Experienced in health insurance navigation and confident in explaining benefits and cost-sharing to patients.
  • Strong critical thinking skills with the ability to triage accurately and escalate appropriately.
  • Comfortable collaborating across teams while maintaining personal accountability for outcomes.
  • Technically proficient in EMR/EHR and CRM platforms (Salesforce, Athena Health) and communication tools (Zoom, Slack, Google Workspace or equivalent).
  • Exceptional verbal and written communicator who can be both efficient and empathetic.
  • Motivated by performance standards and takes pride in responsiveness, reliability, and results.

Compensation Range

$45,000 - $52,000 USD
