Schedule: 9-5pm EST with rotating on-call schedule

Schedule & On-Call Requirements
This role will be part of a four-RN team that provides both daytime clinical coverage and after-hours on-call support. The team operates on a rotating schedule designed to distribute evening, overnight, and weekend call responsibilities across all four RNs.
Standard Daytime Schedule
- Monday–Friday, approximately 9:00 AM–5:00 PM ET
- RNs work their standard daytime schedule during non-call weeks.
On-Call Rotation
- Each RN rotates into the on-call schedule approximately every fourth week.
- During the RN's assigned on-call week, their schedule will shift from the standard daytime schedule to after-hours/on-call coverage.
- Monday–Thursday: On-call coverage from 5:00 PM–9:00 AM ET
- Weekend Coverage: Begins Friday at 5:00 PM ET and continues through Monday at 9:00 AM ET
- After-hours coverage is managed through PagerDuty
This rotation allows the four-RN team to share after-hours responsibilities while providing each RN with approximately three weeks of standard daytime scheduling between on-call rotations. Candidates should be comfortable working a flexible rotating schedule that includes evenings, overnight call, and weekend call approximately once every four weeks.
Role Overview
The GI Registered Nurse plays a pivotal role in supporting clinical excellence, provider productivity, patient safety, and seamless care coordination across all Oshi Health GI service lines. This role provides direct clinical support, triage, and inbox management for Advanced Practice Providers (APPs), Registered Dietitians (RDs), and Behavioral Health (BH) specialists. By managing complex clinical workflows, including urgent symptom escalations, high-volume lab routing, prescription refill protocols, and patient education, the GI RN ensures rapid, high-quality patient care while reducing administrative burden on the broader care team. This role bridges clinical practice with operational efficiency, ensuring standard operating procedures and clinical protocols are executed consistently across all regions.
What You'll Do: Key Responsibilities
Clinical Triage and Symptom Management
- Execute timely asynchronous and synchronous clinical triage for patient-reported symptoms via secure chat, phone calls (TalkDesk), and EHR tasks.
- Act as primary incident lead for Slack-based Urgent Symptom Escalation and 911 Critical Incident workflows.
- Perform clinical risk assessments, apply C-SSRS screening protocols for patients in crisis, stabilize immediate safety concerns, and route appropriately to providers or emergency services.
- Evaluate acute GI complaints (e.g., hematochezia, severe pain spikes, persistent vomiting, fever) and determine appropriate level of care, outpatient directives, or emergency room referrals.
Lab, Imaging, and Diagnostic Management
- Manage, review, and clear high-volume lab and imaging results across general and state-specific clinical inboxes in AthenaOne.
- Review normal lab panels (CBC, CMP, TSH, tTG IgA, CRP, calprotectin, H. pylori antigen) and communicate reassurance and After Visit Summary directives to patients using standardized macros.
- Summarize and highlight abnormal laboratory or diagnostic findings for APP review.
- Provide patient education and manage administrative logistics for specialized testing (e.g., SIBO breath test kits, lab re-orders, facility-specific order matching).
Medication Management and Pharmacy Coordination
- Process medication refill requests and pharmacy inquiries according to established standing orders and refill protocols.
- Manage pharmacy interactions, handle complex prescription submission issues, and resolve coverage/prior authorization barriers.
- Evaluate refill appropriateness, identifying cases that require APP clinical re-evaluation versus those eligible for protocolized fulfillment.
APP Support and Out-of-Office (OOO) Coverage
- Provide comprehensive inbox support for designated APPs during scheduled OOO periods, high-admin volume periods, or caseload surges.
- Review, prioritize, and clear incoming patient cases, orders, and diagnostic results to maintain continuity of care and prevent backlog growth.
- Maintain clear documentation and communication logs to ensure seamless handoffs upon provider return.
Cross-Functional Care Coordination
- Partner closely with Care Coordinators to ensure appropriate routing of clinical versus administrative patient messages.
- Collaborate with RDs and BH providers to support multidisciplinary GI care plans, respecting clinical scope boundaries while assisting with urgent clinical needs.
- Represent nursing workflows in interdisciplinary alignment meetings and quality reviews.
Operations, Process Improvement, and SOP Adherence
- Monitor daily inbox metrics to track handle times, backlog dynamics, and capacity bottlenecks.
- Identify operational inefficiencies, double-documentation practices, or workflow gaps, and actively contribute to process improvement initiatives.
- Participate in testing and adopting new technology, automated lab-routing mechanisms, macro templates, and EHR updates.
What We're Looking For: Qualifications & Requirements
Required
- Active, unencumbered RN license with willingness/ability to hold multi-state licensure as required by service line expansion.
- Demonstrated experience in clinical triage, high-volume patient inbox management, or specialized GI nursing practice.
- Demonstrated clinical expertise across specialized GI pathways, including IBD, IBS, H. pylori protocols, SIBO testing/education, elevated fecal calprotectin, and specialized lab monitoring.
- Proven ability to act as a primary lead for urgent symptom escalations and critical incident workflows (e.g., 911 critical incidents, suicidal ideation/C-SSRS screening, acute crisis stabilization).
- Demonstrates mastery in navigating EHR and communication technology, including AthenaOne, Salesforce, Slack workflows, PagerDuty, and TalkDesk.
- Strong organizational skills to balance competing priorities during high-volume periods or provider OOO coverage surges.
Preferred
- Experience in telehealth operations, asynchronous patient messaging, and multi-channel call management platforms.
- Experience adapting to new software integrations (e.g., Okta SSO, automated lab routing) and updated standing orders.

Compensation Range
$80,000 - $100,000 USD
