The Intake & Discharge Coordinator oversees and facilitates all aspects of the clinical intake and discharge process. This role is highly client-facing and requires strong organizational skills, clear communication, and close collaboration with clinical, admissions, and program teams to ensure a smooth and supportive experience for clients and families.
Responsibilities:
- Coordinate with the Clinical Director, Admissions team, and clinical staff to manage the intake process from start to finish
- Complete intake assessments and ensure all admissions documentation is accurate and complete
- Welcome new clients and orient them to the treatment milieu
- Conduct facility tours, phone assessments, and additional admissions-related tasks as needed
- Communicate professionally and compassionately with clients, families, referral sources, and internal team members
- Support outcomes research initiatives by gathering client feedback, reviews, and assessment data
- Coordinate and ensure completion of all discharge procedures, including returning client belongings and finalizing documentation
- Collaborate with clinical and admissions teams to ensure timely, organized, and supportive discharges
- Support continuity of care through clear communication and documentation
- Maintain accurate and timely clinical documentation and support chart audits as needed
- Participate in treatment team meetings, staff meetings, and in-service trainings
- Assist with onboarding and training for staff involved in intake and discharge processes
- Demonstrate professionalism in appearance, communication, and conduct in alignment with agency values and philosophy
Qualifications
- Bachelor’s degree preferred; equivalent education and relevant experience considered
- Minimum of 2 years of experience in residential treatment, facilities management, or behavioral healthcare preferred
- Minimum of 1 year of experience working with youth and families
- Strong organizational skills with exceptional attention to detail
- Ability to manage multiple priorities in a fast-paced environment
- Excellent verbal and written communication skills, particularly in emotionally sensitive situations
- Proven ability to collaborate effectively with multidisciplinary clinical teams
