
Care Navigator - GUIDE
OUR MISSION
Perry Health’s mission is to deliver high-quality continuous care to chronically ill patients. We started with Diabetes and serving the Medicare population and those populations that don’t typically have access to high quality care between in-person appointments. We’re now working to expand into cognitive health, supporting those with cognitive decline and their caretakers.
Over 130M Americans suffer from chronic disease and that number is expected to reach 170M by 2030. What is even more staggering is that 85% of seniors suffer from a chronic disease. The problem as we see it is that in person care is not adequate for patients living with chronic disease, leading to poor outcomes and increased cost. This is why we created Perry.
Our care team is currently made up of physicians, nurse practitioners, registered nurses, registered dietitians, health coaches, clinically licensed social workers and now we are looking forward to expanding our support to care navigation. Our program creates real outcomes for our members and thousands of dollars in savings per patient to the healthcare system. And we’re just getting started.

ABOUT THE ROLE
We’re looking for an experienced Registered Nurse to initiate our Care Navigation function within our Medical Practice, specifically to support our dementia care programming. We’re working to be CMS GUIDE-accredited and will rely heavily on this role for care coordination, education and support efforts for patients and caregivers alike. KEY RESPONSIBILITIES
- Care Coordination
- Both internally to our care team and externally to any providers patients may be seeing in person, the coordination of shared information will be paramount; the ability to effectively communicate, investigate and regurgitate is highly important for this role.
- Care Education
- The scope here can range from anything regarding medication education to lifestyle recommendations, from internal Perry protocols to GUIDE-suggested procedures; a strong candidate will confidently and accurately educate patients and caregivers or refer to appropriate parties where needed.
- Care Navigation
- Support patients & caregivers with any clinically recommended referrals; a successful candidate will help patients identify in-network providers, schedule appointments, arrange transportation and share and receive information in a closed loop fashion.
- Administration
- This role may come with ad hoc administrative responsibilities to ensure patients are meeting CMS requirements & continue to qualify for the GUIDE program. A candidate with attention to detail and efficiency-finding tendencies will be needed to perform the role well.
QUALIFICATIONS
- 5+ years of clinical care navigator experience
- 3+ years of working with patients and caregivers with cognitive decline
- An unrestricted multi-state compact RN license, and additional Massachusetts license a plus
- A desire to work in an early startup environment
- In-depth knowledge of the health plan/payor ecosystem, namely Medicare, Medicaid and experience working with the GUIDE model a big plus
- Strong interpersonal communication skills
- Bilingual (Spanish-speaking) a plus
