OUR MISSION
To transform cognitive care by building a trusted, scalable system that combines world-class clinics, technology-driven care navigation, and compassionate support for patients and their caregivers. We aim to deliver measurable human outcomes, ensuring every patient receives the care they need, when they need it, with dignity and trust.
ABOUT THE ROLE
We’re seeking a Nurse Practitioner who is passionate about caring for patients with memory and cognitive concerns. While prior neurology or dementia care experience is a plus, it is not required. We welcome candidates from family medicine, internal medicine, geriatrics, and other primary care backgrounds who are eager to expand their expertise into memory and cognitive health.
To support this, Mindspan provides in-depth, in-house training developed by our neurologists. You will gain hands-on experience, ongoing mentorship, and access to educational resources to build confidence in dementia-focused care.
In this role, you will be a frontline provider of cognitive and memory care at our Danvers clinic and through telemedicine visits. You’ll conduct cognitive and functional assessments, develop tailored care plans, monitor changes over time, and provide compassionate education and support to patients and their families. This is an opportunity to practice at the leading edge of dementia and memory care in a program designed exclusively for this population, where you’ll play a vital role in guiding patients and caregivers throughout their journey while also supporting the clinical organization with escalations as they arise.
KEY RESPONSIBILITIES
- Provide high-quality, patient-centered care for individuals with cognitive impairment, dementia, Alzheimer’s disease, and other memory-related conditions (in-person and via telehealth)
- Conduct comprehensive cognitive assessments, review medical history, labs, imaging, and medications
- Develop, implement, and monitor individualized treatment and care plans in collaboration with neurologists and interdisciplinary team members
- Support patients and caregivers in understanding diagnoses, treatment options, and disease progression
- Serve as a point of escalation for clinical issues within the practice
- Maintain accurate, compliant documentation within the electronic health record (EHR)
- Stay current with clinical guidelines and best practices in neurology and dementia care
QUALIFICATIONS
- Education: Bachelor of Science degree in Nursing (BSN) & Master of Science in Nursing (MSN)
- License: Active, unencumbered state license in Massachusetts
- Experience: Minimum 2–3 years in primary care, family medicine, internal medicine, geriatrics, or another patient-facing specialty
- Strong clinical judgment, excellent communication skills, and ability to work collaboratively in a multidisciplinary team
- Passion for advancing dementia care and supporting both patients and caregivers
