Remote Accountability Coach (Contracted)
THE HIGHLIGHTS
- Anticipated Start Date: Late Spring – Summer 2026
- Role Type: Contracted, 1099
- Payment Terms: Net 30
- Licensure/Certification: NBC-HWC, OR LPN or higher clinical licensure required
- Schedule: 1099 flexibility; you set your own hours, ideally 20+ hours of weekly availability.
- Current Demand: Monday–Friday, 10 a.m.–7 p.m. ET
- Requirement: Provide availability for the next two months by the 15th of each month.
- Compensation: Budget for an average of $30/hr (see Compensation & Payment Terms below)
- Remote Requirements: Must reside in the United States with consistent access to a clean, quiet workspace; solid internet connection; and proficient technical experience.
OUR MISSION
Perry Health is rethinking chronic care through a fully remote, continuous care model. We provide expert diabetes education and clinical management for Medicare patients, delivered by a comprehensive team of specialized CDCES Registered Nurses, Registered Dietitians, and Accountability Coaches.
Our model integrates high-level clinical expertise with real-time data from cellular-connected glucometers and advanced CGM management. By bridging the gap between physician visits with proactive, data-driven support, we deliver life-changing outcomes for our members.
ABOUT THE ROLE
We are seeking highly enthusiastic, self-motivated professionals passionate about remote care delivery. In this role, you will support diabetes education and management for our members through lifestyle modifications and high-frequency support. You will partner with clinical professionals to ensure members receive the attention and education they need to lead healthier lives.
KEY RESPONSIBILITIES
- Serve as an expert diabetes health coach, supporting individuals to achieve the best health outcomes possible.
- Engage members with empathy and care via telephone calls and text messaging.
- Deliver education and personalized guidance according to Perry Health protocols to help members better self-manage their condition and adhere to their care plan.
- Recognize immediate care needs and triage/address them accordingly to Perry Health clinicians.
- Collaborate with the broader care team — including RNs and RDs — to reinforce clinical goals and SMART goal progression.
- Adhere to Perry Health policies, procedures, and standard operating procedures to ensure ongoing clinical quality and compliance.
QUALIFICATIONS
- Eligibility: National Board-Certified Health & Wellness Coach (NBC-HWC), OR LPN or higher clinical licensure.
- Knowledge: Chronic disease management experience, especially diabetes — preferred, not required. Substantial knowledge of the healthcare industry. Familiarity with behavior change theory and continuous glucose monitors (CGMs) is a plus.
- Skills: Motivated learner, willing to acquire proficiency with the necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to support sustainable behavior change.
- Traits: Creative thinking, an eye for detail, ability to work independently, a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
- Language: Bilingual (Spanish) is a plus.
COMPENSATION & PAYMENT TERMS
Contractors are paid on a monthly basis following Net 30 payment terms.
- Clinical Pay: $15.00 per Clinical Encounter. An encounter is defined as a successful call with a member, with at least 20 minutes of total engagement time — primarily spent in live conversation, with chart review time also counting toward this total.
- Example: Calls are scheduled in 30-minute intervals. If you complete two calls within an hour and spend at least 20 minutes with each member, your total compensation for that hour would be $30.
- Non-Clinical Pay: $22.00/hour flat rate for required meetings and training.
