Chronic Care Coordinator – Partners In Nephrology & Endocrinology

Requirements:

• Certified Medical Assistant or Licensed Practical Nurse with 5 to 10 years of experience

• Strong multitasking and problem-solving abilities

• Excellent written and verbal communication skills

• Experience in endocrinology required

Preferred Qualifications:

• Experience with Epic 

• Background and experience in nephrology, dialysis, or care coordination

Compensation & Benefits:

• $18 to $19.50 per hour, full-time

• Monday-Friday, 8:00 AM - 5:30 PM

About the Role:
Phamily is assisting Partners In Nephrology & Endocrinology in hiring a Chronic Care Coordinator to support nephrologists and advanced practitioners in managing chronic care patients. Using the Phamily Chronic Care Management platform, you will help ensure timely access to preventive services, bridge communication between patients and providers, and work to prevent unnecessary hospitalizations.

Responsibilities:

• Coordinate chronic disease care using the Phamily platform

• Support nephrologists and advanced practitioners in managing chronic care patients

• Facilitate communication between patients and care teams

• Assist in improving patient access to preventive services and chronic disease treatment
