Triage Coordinator - Loganville, GA
Position Summary:
The Triage Coordinator serves as the central point of contact for managing and prioritizing patient care inquiries, clinical messages, and urgent dermatologic concerns. This role supports the clinical team by performing initial patient assessments via phone or electronic communication and directing appropriate care pathways. The Triage Coordinator ensures timely responses to patient needs while maintaining high standards of communication, documentation, and customer service.
Essential Duties and Responsibilities:
To perform effectively in this role, the candidate must fulfill the following duties with or without reasonable accommodations:
- Respond to incoming patient calls, messages, and portal inquiries related to clinical concerns, medication requests, post-procedure symptoms, and follow-up care.
- Assess the urgency of patient issues and escalate time-sensitive concerns to providers or nursing staff as appropriate.
- Document all triage interactions thoroughly in the electronic health record (EHR).
- Coordinate with providers to communicate instructions, prescriptions, or follow-up plans to patients.
- Assist in scheduling urgent or follow-up appointments based on triage outcomes.
- Provide patient education related to post-procedure care, prescription use, and dermatology-specific conditions within the scope of training.
- Maintain triage protocols and ensure adherence to clinical guidelines and HIPAA standards.
- Serve as a liaison between patients, providers, pharmacies, and outside specialists or laboratories when needed.
- Collaborate with medical assistants and front office staff to ensure seamless care coordination.
- Participate in quality improvement initiatives and training related to triage and dermatology care best practices.
Required Knowledge, Skills, and Abilities:
This role requires a proactive, dependable, and detail-oriented individual with the ability to manage multiple tasks in a dynamic healthcare environment. The ideal candidate demonstrates a strong commitment to patient care and operational excellence.
- High school diploma or equivalent.
- Minimum of 2 years of experience in a clinical or healthcare setting, preferably in dermatology or a specialty clinic.
- Knowledge of basic medical terminology and dermatologic procedures.
- Proficient in electronic health records (EHR) and clinical documentation.
- Strong communication, problem-solving, and organizational skills.
Preferred:
- Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), or Registered Nurse (RN).
- Prior experience with dermatology practice management systems (e.g., EMA, Modernizing Medicine).
- Bilingual (Spanish/English) is a plus.
