Licensed Practical Nurse (LPN) - North Falmouth, MA
Position Summary:
As a Licensed Practical Nurse (LPN), you will be a vital member of the healthcare team, ensuring efficient and compassionate patient care. Your responsibilities include assessing patient needs, coordinating appropriate care plans, communicating effectively with healthcare providers, and delivering their instructions to patients. Your nursing expertise and professionalism are critical to upholding the highest standards of patient care.
Essential Functions:
To perform effectively in this role, the candidate must fulfill the following duties with or without reasonable accommodations:
- Open and close the triage queue at designated times as directed by management.
- Promptly answer and respond to incoming phone calls and monitor refill and patient queues for messages.
- Collect detailed patient information, including symptoms and current medical conditions.
- Assess the urgency of patient needs and determine the appropriate timing and provider for care.
- Accurately document medical advice and the patient’s response in the electronic medical record (EMR).
- Create comprehensive telephone encounter notes capturing all pertinent patient information.
- Communicate lab and pathology results to patients promptly and professionally.
- Refill medications following provider-specific protocols and document actions in the EMR.
- Assist in completing prescription prior authorizations as needed.
- Monitor and respond to patient portal messages in a timely manner.
- Schedule appointments within the practice or with external providers as directed.
- Work collaboratively with staff and healthcare providers to address patient concerns and ensure continuity of care.
Knowledge, Skills, & Abilities:
- Licensure and Certifications:
- Valid LPN license.
- Current CPR certification.
- Preferred Education:
- Bachelor’s degree is preferred.
- Technical Proficiency:
- Familiarity with EMR systems and other healthcare technologies.
- Interpersonal Skills:
- Strong communication skills, both verbal and written, with a focus on compassionate patient care.
- Organizational Skills:
- Ability to prioritize tasks effectively and manage time in a fast-paced environment.
