At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they’re most comfortable—creating a seamless, convenient, and personalized process for seniors and their caretakers.
The Opportunity
The Primary Care Coordinator serves as the central point of contact for our primary care geriatric care team, managing 500-600 patients alongside nurses and Primary Care Providers. The role focuses on coordinating patient care, maintaining relationships with senior living facilities, and ensuring excellent healthcare delivery through effective communication and documentation.
***This role offers a flexible hybrid arrangement with 1-2 days per week in-office, plus additional on-site coverage as needed for clinical operations.***
Key Responsibilities:
- Serve as a primary contact for patients, families, and providers
- Schedule and coordinate medical appointments
- Manage patient documentation and EMR updates (Epic experience a plus)
- Process urgent calls and tasks
- May participate in mandatory after-hours shift rotation in the future
- Handle communications including those via phone, email, text, fax, portal, etc.
- Coordinate with community partners and specialty providers
- Facilitate new patient onboarding
Success will be measured in areas such as:
Inbound Phone Calls:
-Answering calls quickly; aiming for short caller wait time
Faxes:
-Processing of faxes
Task Completion:
-Messages and email responsiveness
-Completion of STAT tasks within 24 hours
-Proactively contacting newly enrolled patients within 24 hours to schedule a welcome visit
-Completion of visit reminder calls
Voicemails:
-Closing, resolving, and returning voicemails
Patient Care Management:
-Ensuring accurate documentation of chronic care management
-Providing ~6 hours per day of care coordination using our custom logging software
-Assisting with improvement projects related to quality and efficiency
-Achieving high patient satisfaction survey scores
Attendance:
-Reliability
Requirements:
- High School Diploma (some college preferred)
- Basic understanding of Primary Care Operations
- Medical Assistant Certification preferred
- Reliable internet and HIPAA-compliant workspace; you must have a confidential space to ensure HIPAA compliance and to preserve patient PHI
- Comfort with healthcare technology platforms
- Ability to thrive in a fast-paced, changing environment
- Attendance is critical in this role to ensure quality patient care
- Ongoing Regulatory Requirement: Must not be on any exclusion or debarment from
participation in Federal Health Care Programs at any time and must remain in good standing
with government regulators such as the OIG, CMS, etc.
Physical Requirements:
- Ability to remain seated in a stationary position for prolonged periods
-Good manual dexterity is required to use standard office equipment (e.g., computers, laptops, and mobile devices)
- High proficiency with computers and mobile devices
