NP Clinical Leader | Bay Area
The Opportunity
We're looking for a Clinical Director (NP) to join our Bay Area team — someone who combines strong clinical skills with a real interest in leading and developing other providers. This is a hybrid clinical and leadership role. You'll carry a small patient panel while managing and mentoring a team of 6–8 NPs and PAs, owning their performance, growth, and day-to-day support.

Clinical Responsibilities
- Establish meaningful patient relationships through comprehensive Welcome Visits and develop personalized Care Plans covering goals, diagnoses, medications, and advanced care planning.
- Ensure timely completion of Annual Wellness Visits focused on preventive care and comprehensive documentation.
- Provide seamless clinical coverage during provider absences, maintaining continuity of care.
- Conduct patient assessments, medication management, and follow-up care focusing on geriatric-specific needs.
- Respond promptly to urgent medical and behavioral health concerns to prevent unnecessary hospitalizations.
- Conduct post-discharge visits for patients transitioning from acute or skilled nursing facilities.
- Adapt workflows to accommodate urgent care needs while maintaining quality preventive services.
Leadership Responsibilities
- Lead and develop a team of 6-8 Nurse Practitioners and Physician Assistants through regular performance reviews, mentorship, and professional development
- Ensure providers meet key metrics, including patient visit targets, documentation timeliness, and care gap closures.
- Optimize provider schedules and panel assignments.
- Drive community growth by attending pipeline meetings, leading implementation kickoffs, and building relationships with senior living facility staff.
- Serve as the primary point of contact for community partners, addressing concerns promptly and fostering collaborative relationships.
- Lead quality improvement initiatives to enhance patient care and provider efficiency.
- Participate in the hiring, onboarding, and training new clinical team members.
- Collaborate with cross-functional teams to improve care delivery systems and clinical workflows.
What We’re Looking For
- Active CA APRN license; California 103 NP license preferred
- Active unrestricted DEA with schedules 2, 2N, 3, 3N, 4, 5
- Master's degree in Nursing with Family or Adult-Gerontology specialization preferred
- Minimum 3 years of primary care experience; geriatric, community-based, or senior living experience strongly preferred but not required
- Minimum 2 years of direct people management experience supervising Nurse Practitioners, Physician Assistants, or similar clinical staff — including performance reviews, managing to metrics, and hiring decisions; informal mentorship or charge nurse experience does not meet this requirement
- Demonstrates exceptional clinical acumen, including advanced diagnostic skills and comprehensive medical knowledge
- Exceptional clinical assessment skills with the ability to develop comprehensive care plans and chronic disease management protocols
- Demonstrated ability to work autonomously in fast-paced, multi-site healthcare environments

Travel is required for this role. You must have the ability to drive between partner facilities regularly.
The base salary range for this role is $175,000 – $205,000, commensurate with experience and leadership background, plus a performance-based bonus that can increase total compensation. Final compensation will be determined based on clinical depth, years and scope of management experience, and overall fit for the role. Total compensation also includes equity and full benefits.
