Lead, Community Outreach and Care Coordination (Bilingual English/Spanish)

About the Role  

We're looking for a Community Outreach and Care Coordination Lead to launch and run our Upstate New York field team, starting in the Finger Lakes. This is a player-coach role: you'll supervise four Community Outreach Coordinators across 14 counties while doing your own screening, navigation, and enrollment work in the field — roughly half your time on each. You'll own our highest-value regional partnerships, from health systems and FQHCs to food banks and community organizations, and serve as the local escalation point for our SCN partner. You'll report to the NY Regional Manager, Community Outreach. 

What You’ll Do 

  • Supervise, coach, and support four coordinators across the Finger Lakes, including weekly check-ins and field ride-alongs 
  • Build and manage the territory schedule — recurring county days, events, and travel routes across 14 counties 
  • Conduct HRSN screenings, eligibility assessments, and navigation in FindHelp, and enroll members in food boxes and ECM in the field 
  • Own the region's highest-value referral partnerships: health systems, FQHCs, Health Homes, behavioral health providers, food banks, and community organizations 
  • Review your team's screenings, navigation records, and case notes for quality and compliance 
  • Connect members to housing, transportation, and behavioral health resources, and send warm leads to our call center 
  • Report weekly on screenings, enrollments, leads, and partner pipeline, and keep activity current in HubSpot 

What You Bring 

Required 

  • Bilingual in Spanish and English 
  • 3+ years of case management, care coordination, or community health work with Medicaid populations 
  • 1+ year supervising or leading staff 
  • Based in or near Rochester, with a valid NYS driver's license, reliable vehicle, and willingness to drive long rural distances — some county days are up to two hours out 
  • Strong documentation habits and comfort with data and weekly reporting 

Preferred 

  • Experience with social needs screening and referral in FindHelp, Unite Us, or a similar platform 
  • NYS CHW certification, or Health Home or ECM care management experience 
  • Knowledge of Rochester and Finger Lakes community resources 
  • Bachelor's degree in social work, public health, human services, or a related field 

Compensation & Benefits 

The expected salary range for this role is $65,000 - $75,000 annually, plus performance bonus and benefits. Compensation for this role will depend on various factors, including qualifications, skills, and location.
