About the Team & Opportunity:
Quartet is looking for a Therapist in the New Jersey/Pennsylvania area who is passionate about providing holistic care to those with serious mental illness. Our Therapists are critical members of our multidisciplinary team, responsible for managing whole-person care for adult patients with serious mental illness, both virtually and in the home.
Accountabilities:
- Administer evidence-based therapy to engaged patients as determined by Care Plan, facilitating patient engagement and follow-up care
- Develop a strong rapport with a diverse set of patients (socioeconomic status, age, sexual orientation etc.) both in-person and via telehealth, supporting patient engagement in the program
- Assist with Medical and Behavioral Health assessment completion
- Participate in Patient Rounding meetings led by the Nurse Practitioner to drive individualized care planning using appropriate diagnostic criteria
- Facilitate treatment plan changes or referrals needed for patients who are not improving as expected in consultation with the patient’s Nurse Practitioner
- On point for escalations for behavioral care needs / issues
- Willingness to assist with after hours call coverage
- Uphold clinical customer service through the delivery of quality, empathetic care
- Document concurrently all clinical findings and services provided in the electronic medical record as required by compliance standards
Minimum Qualifications:
- Active licensed therapist in good standing with the state of New Jersey
- Experience in screening, assessment and treatment for mental health and/or substance abuse disorders
- Strong working knowledge of behavioral health diagnoses and evidence-based psychosocial treatments for mental health and / or substance abuse disorders
- Ability to travel locally at least 20% of the time to see patients in the home and community
- Have a valid driver’s license, active auto insurance, have access to a car and willingness to drive to the patient location
- Excellent written and verbal communication with an emphasis on clarity and compassion
- Strong computer skills, including experience working with Electronic Medical Records (EMR).

Preferred Qualifications:
- Experience in home-based services, complex care cases, or caring for patients with co-occurring substance use disorders is preferred
- Experienced in caring for patients with Serious Mental Illness
- Bilingual candidate (Spanish and/or other languages)
- Previous telemedicine experience and skilled at using online tools and technology to deliver care and communicate with patients
