POSITION TITLE: Case Management Supervisor, Coordinated System of Care (AB 2083) (Foster Care) & Specialized Caseloads  

SALARY RANGE: $3,539.34 – $5,229.23 Biweekly; $92,022.84 - $ 135,959.98 Annual

REPORTS TO: Associate Director of Client Services (TBD)

LOCATION: May report to either the San Leandro or Concord offices.

The Regional Center of the East Bay (RCEB) is one of 21 Regional Centers that are community-based, private non-profit corporations under contract with the California Department of Developmental Services. RCEB provides intake, assessment, diagnosis, and coordinates community-based services for over 25,000 children and adults with developmental disabilities in Alameda and Contra Costa counties.

SUMMARY OF THE POSITION:

This position was established as a permanent position enacted by Assembly Bill (AB) 2083 adding legislative intent to develop a coordinated, timely, and trauma-informed system-of-care approach to serving foster children and youth who have experienced severe trauma. AB 2083, known as the Children and Youth System of Care, requires each county to develop and implement a memorandum of understanding (MOU), setting forth the roles of regional centers and other entities that serve children and youth in foster care who have experienced severe trauma by addressing systemic barriers to the traditional provision of interagency services.

AB 2083 requires the establishment of a state level Joint Interagency Resolution Team consisting of representatives from the Department of Social Services (CDSS), Department of Health Care Services, Department of Education and the Department of Developmental Services. As specified in W&I Code sections 16521.6(b)(1)(B)(i) and (ii), the Joint Interagency Resolution Team is required to ensure that a process is developed and implemented for counties and local partners that are parties to the MOU to request and receive interdepartmental technical assistance regarding the development of the MOU and to identify and secure the appropriate level of services.

Children and Youth System of Care Coordinator will have an important role in coordinating with local partners and securing appropriate level of services for children and youth in foster care.

In addition, this position will supervise a small unit of case managers responsible for a Specialized Caseload serving individuals with complex needs, in accordance with Welfare & Institutions Code (WIC) § 4640.6. The Supervisor will perform a full range of administrative and supervisory functions to ensure the delivery of high-quality case management services. Responsibilities include oversight of case documentation, monitoring consumer contacts, and ensuring compliance with regulatory requirements. This position will oversee case managers, including one for Porterville Developmental Center (PDC), and three specialized complex-needs caseloads, each with a 1:25 case manager-to-consumer ratio, focused on crisis stabilization, primarily serving adults, with a mix of children and youth as needed.

This role also involves facilitating and overseeing the development and implementation of Individual Program Plans (IPPs) for assigned consumers, evaluating the effectiveness of services provided, and stepping in to support case management functions as needed.

A key focus of this position is supervising case managers whose primary responsibility is to monitor and assist consumers transitioning from Institutions for Mental Disease (IMDs), Stabilization Training Assistance Reintegration (STAR) facilities, and/or correctional settings. The Supervisor will participate in coordination meetings with the Department of Developmental Services (DDS) to provide updates on these transitions and will ensure appropriate supports are in place to maintain the health and safety of each consumer throughout the process.

Activities include, but are not limited to:

  1. Participating in the development and implementing MOUs in the regional center’s catchment area, as specified in W&I Code section 16521.6(a)(1).
  2. Collaborating and sharing information with counties, state agencies and other specified entities to identify and coordinate services for youth in foster care and youth served by regional centers, who have experienced severe trauma as specified in W&I Code section 16521.6(a)(3)(A).
  3. Identifying and coordinating available placement and service options for youth in foster care served by regional centers as specified in W&I Code section 16521.6.
  4. Participating on interagency leadership and placement teams as specified in W&I Code sections 16521.6(a)(2)(A) and (B).
  5. Providing expertise and guidance to regional center staff on service coordination for children, youth in foster care and adults with complex needs served by regional centers, who have experienced severe trauma.
  6. Interacting with county social workers to identify and reduce barriers for timely out-of-county transfers, intake, and referral process for infants, toddlers, and youth involved or at-risk of being involved with child welfare per W&I Code section 4643.5.
  7. Providing ongoing training, outreach, and systems navigation to regional center staff and system partners regarding regional center intake processes and services.
  8. Providing the Department of Developmental Services (Department) with data, implementation updates and recommendations.
  9. Participating in state coordinated trauma-informed behavior support trainings.
  10. Engaging and contributing to Department coordinated technical assistance and collaborative opportunities.
  11. Providing direct supervision to case managers responsible for specialized caseloads.

MINIMUM QUALIFICATIONS

  1. Master’s degree in a human services field (or) Bachelor’s degree in human services with four years paid experience as a case management supervisor, experience working with children and/or young adults ages 3-22 with intellectual and developmental disabilities.  
  2. Demonstrated knowledge and skills in program management, organization, and staff development.
  3. Outstanding interpersonal skills in dealing with diverse peoples in a wide variety of situations.
  4. Ability to work collaboratively across the organization within a team approach.
  5. Excellent writing skills, particularly in report writing and formulating recommendations for action.
  6. Demonstrated knowledge and skills in the field of developmental disabilities and related fields, particularly in innovative and forward-looking approaches to providing services and supports to individuals with developmental disabilities and their families.
  7. Automobile and valid driver’s license.
  8. Have an understanding of Person Centered Thinking.
  9. Demonstrate cultural competency and sensitivity and an ability to work effectively with culturally diverse populations.
