Are you a skilled and compassionate Infusion RN looking to make a meaningful impact in home health? We’re seeking an experienced nurse to join our team for per diem home health visits, providing top-quality care directly in patients' homes across Los Angeles County. If you thrive in a flexible, dynamic environment and are passionate about delivering exceptional patient care, we want to hear from you! 

What You’ll Do: 

• Infusion Care: Administer infusions, including IV, Port, and PICC Line care, as well as other injections in a home setting. 

• Patient Care Coordination: Oversee the care process from start to finish, including completing SOC/ROC/Recertification/DC/Follow-up documentation. 

• Comprehensive Clinical Skills: You’ll also be involved in wound care, ostomy care, blood draws, and more, ensuring patients receive comprehensive care. 

• Drive with Purpose: Travel to various patient locations within LA County, providing personalized care where it’s needed most. 

 

Experience & Skills: 

• Proven experience in IV infusion (peripheral and PICC line) 

• Expertise in Port line care and PICC line management 

• Knowledgeable in wound and ostomy care 

• Comfortable with computer-based documentation (experience with WellSky/Kinnser is a plus) 

Qualifications: 

• Current California RN license 

• CPR Certification (current) 

• TB Test and Physical Exam (current) 

• Valid Driver’s License and Auto Insurance
