The Opportunity

Rhode Island Primary Care Physicians Corporation (RIPCPC) is seeking a Director of Nursing to lead and oversee our ambulatory and practice-based care management programs, including Complex Care, Transitions of Care, and Chronic Care Management. This full-time leadership role reports to senior leadership and plays a critical part in achieving our quality, cost, and patient experience goals. You'll guide a multidisciplinary team, ensure compliance with NCQA standards, and foster strong relationships with stakeholders across the healthcare continuum. What you’ll do

  • Design and implement care management programs that improve patient outcomes, reduce healthcare costs, and enhance the provider and patient experience
  • Provide oversight and mentorship to a team of Nurse Care Managers and LPNs, including remote staff in physician practices across Rhode Island
  • Conduct regular audits to ensure adherence to NCQA and payor standards, as well as internal care management protocols
  • Supervise the Manager of Care Management in the implementation and performance tracking of metric-driven care programs
  • Lead all administrative responsibilities such as timecards, PTO, onboarding, performance reviews, and re-training
  • Collaborate with directors of Pharmacy, Behavioral Health, and Quality & Performance to support integrated care delivery
  • Analyze and report daily Key Performance Indicators such as readmissions, ED visits, and care management reach/engagement
  • Represent the Care Management team in Joint Operating Committees with health plans and ACOs
  • Evaluate and recommend new programs and services to improve quality and efficiency of care
  • Facilitate the integration of care teams within participating physician practices, ensuring strong communication and performance monitoring

Who you are

  • Bachelor’s degree in Nursing (required); Master’s degree in Nursing or Healthcare Administration (preferred)
  • Active Rhode Island RN license (required)
  • 3+ years of experience leading a nursing team in a metrics-driven, multi-disciplinary environment (required)
  • Experience with NCQA Care Management standards (preferred)
  • Strong organizational, communication, and leadership skills
  • Ability to work effectively with remote and in-person teams
  • Proficient in analyzing healthcare data and implementing process improvements
  • Knowledge of value-based care models and care coordination best practices (preferred)

Schedule:

  • · Monday to Friday (some night and early morning meetings)
  • · Role is based in Cranston, RI. with some flexibility for remote work.

Salary range

$110,000 - $150,000 USD
