
Full Time Discharge Planner
Position: Discharge Planner
Location: Crownsville, MD | Adolescent Residential Treatment Center
Schedule: Full Time | Monday to Friday 9:00am to 5:00pm
Compensation: $22-24 Per Hour + Full Time Benefits
About The Role
The Discharge Planner plays a vital role in ensuring adolescent clients transition smoothly from residential care to ongoing treatment and community-based resources. This position is responsible for discharge planning, developing aftercare summaries, coordinating follow-up care, and maintaining strong relationships with referents, treatment centers, and community providers
Key Responsibilities:
- Lead discharge planning, including aftercare coordination and documentation of discharge summaries.
- Collaborate with clinical teams to assess client needs and develop individualized aftercare plans.
- Serve as a liaison between clients, families, referral agencies, and community providers to ensure continuity of care.
- Participate in clinical team meetings to discuss treatment progress, challenges, and discharge planning strategies.
- Adhere to policies and procedures to ensure compliance with regulatory and quality standards.
- Work closely with admissions, clinical, and outreach teams to maintain strong communication and care coordination.
- Educate clients and families on available aftercare options, addressing potential barriers to continued treatment.
- Maintain organized and accurate records of all discharge planning activities.
Education & Experience Requirements:
- Education: Bachelor’s degree in behavioral health science (Social Work, Psychology, Marriage and Family Therapy, or related field) or Education preferred.
- Experience:
- Previous experience in substance abuse treatment required.
- Experience in a detox or residential treatment facility preferred.
- Discharge planning experience highly desirable.
- Business development experience is a plus.
- Clinical Coordination: Ability to assess client needs, coordinate treatment team input, and develop effective aftercare plans.
- Communication & Collaboration: Strong verbal and written communication skills to engage with clients, families, and external partners.
- Crisis Management & Problem-Solving: Ability to navigate complex situations, address challenges, and provide appropriate aftercare solutions.
- Organizational Skills: Detail-oriented with strong documentation, scheduling, and follow-up abilities.
- Public Relations & Advocacy: Capable of building relationships with community providers, advocating for client needs, and ensuring continuity of care.
