Position Overview:
We are seeking a highly skilled and empathetic Palliative Specialist (PA or NP) to join our team. In this role, you will provide palliative care to patients, focusing on symptom management, pain relief, and improving the quality of life for individuals with serious health conditions. This position offers a blend of telehealth and in-person patient care, with the opportunity to collaborate closely with a multidisciplinary team of healthcare professionals.
Key Responsibilities:
- Provide comprehensive palliative care consultations to patients with chronic or serious illness.
- Develop and implement individualized care plans focusing on symptom management, pain relief, and emotional support.
- Collaborate with physicians, nurses, social workers, and other healthcare providers to ensure comprehensive, team-based care.
- Educate patients and their families about illness progression, treatment options, and goals of care.
- Monitor patient outcomes and adjust care plans as necessary.
- Participate in interdisciplinary team meetings and case reviews.
Qualifications:
- Current, valid certification as a Physician Assistant (PA) or Nurse Practitioner (NP) with specialization in palliative care, geriatrics, or related field.
- Certification such as Advanced Certified Hospice and Palliative Nurse (ACHPN)® is preferred.
- Minimum of 1-3 years of experience in palliative care, hospice, or chronic disease management.
- Strong communication skills with the ability to provide compassionate care and counseling to patients and families.
- Ability to work independently, manage a diverse caseload, and coordinate care in a remote setting.
- Experience in telehealth is a plus.
- Current licensure to practice in the state of California (or eligibility to obtain).
