About the Role:

In this position, you’ll provide in-home primary care to patients across your region, including:

  • Conducting comprehensive assessments and developing care plans

  • Managing chronic conditions and performing routine medical procedures

  • Collaborating with interdisciplinary teams to ensure high-quality outcomes

  • Building meaningful relationships with patients and families

Travel Requirement:
This is a travel-based position that requires you to be in the Chico area on a weekly to bi-weekly basis. A travel stipend will be provided for visits to this region. When not in Chico, you’ll serve patients in surrounding areas based on need and availability.

Pay: $150,000.00 – $300,000.00 per year
