Position Overview
As a Wellness Care Navigator, you’ll wear two hats: one in the senior living community, and the other as a care coordinator. You'll be the go-to support for residents and a vital link between patients, families, and providers. From helping with telehealth visits to managing care plans, your role is key in ensuring patients feel supported and connected.
What You’ll Do
Community-Based Responsibilities:
Here’s what your day might look like:

Be a familiar, trusted face in the community, building strong relationships and offering support

Meet with local agencies to build partnerships that benefit patient care

Help coordinate care and streamline communication between families and healthcare providers

Support provider introductions in the community

Document and share patient care plans with providers

Assist residents with telehealth visits by setting up technology and providing in-visit support

Conduct daily wellness check-ins for patients on services

Offer companionship and emotional support to residents

Join in on community and company events

Share practical health tips and help patients follow their care plans

Report any concerns or urgent findings to providers

Help train and mentor new team members
Care Coordination & Navigation Responsibilities:

Serve as the main point of contact for a group of patients, supporting them throughout their care journey

Create and manage personalized care plans in collaboration with providers and care teams

Track patient progress and ensure they’re following care recommendations

Educate and support patients and families on managing chronic conditions like dementia or diabetes

Schedule referrals, diagnostic tests, and appointments
-
Advocate for patient needs and connect them with social services or other resources
-
Encourage goal setting and support behavioral changes through shared decision-making

Keep accurate, up-to-date records in the care system

Join care team meetings and contribute to ongoing quality improvements
Who You Are
Must-Haves | Preferred |
---|---|
18+ years old | Medical Assistant diploma |
High school diploma or GED | Experience in patient care or case management |
Friendly, professional, and dependable | Licensed phlebotomist (bonus) |
Strong communicator and multitasker | Experience with dementia care |
Comfortable with basic tech and telehealth tools | Background in senior living or long-term care |
Able to stand for long periods and travel locally | Spanish speaking (a plus) |
Available for occasional community events after hours |
