We are seeking a compassionate Field Medical Assistant to join our dedicated care team in Orange County. This field-based role is essential in coordinating care and providing meaningful support to seniors and their families. The ideal candidate is patient-focused, detail-oriented, and thrives in a collaborative healthcare environment. Key Responsibilities

 

  • Conduct comprehensive patient assessments and develop individualized care plans.

  • Coordinate referrals, follow-ups, and care updates with patients, families, and providers.

  • Manage communications via phone, email, and internal systems to ensure timely updates.

  • Maintain thorough, accurate, and compliant documentation in care management systems.

  • Serve as a liaison between patients, providers, and care partners to support continuity of care.

  • Monitor patient needs, evaluate progress, and adjust care plans as appropriate.

  • Educate and support patients and families regarding treatment options, resources, and care processes.

  • Conduct diagnostic evaluations and contribute to treatment planning.

  • Provide both administrative and clinical support to the wrap around medical team.

  • Participate in quality improvement initiatives and contribute to team and organizational goals.

 

Perform other duties as assigned to support patient care and team operations


What We’re Looking For

  • Medical Assistant certificate (Required)

  • Excellent phone communication and etiquette

  • Friendly, positive attitude and ability to build trust quickly

  • Strong organizational and multitasking skills

  • Experience working with seniors or in geriatric care

  • Comfortable collaborating with multiple providers and care teams

  • Tech-savvy and able to learn care coordination platforms

  • Reliable, professional, and compassionate in all interactions
