About the Role

Lykos Medical as part of Brigade Health is seeking a Primary Care Physician (MD/DO) to provide supervisory and clinical oversight for Long-Term Care (LTC) and Skilled Nursing Facilities (SNF) across rural Southeast Colorado.

This role is ideal for physicians seeking schedule flexibility, autonomy, and the opportunity to support underserved senior communities.

Responsibilities

  • Provide supervisory oversight and monthly inperson rounding
  • Manage chronic and complex geriatric patients in LTC/SNF settings
  • Collaborate with facility nursing and administrative teams
  • Document care in EMR in a timely manner
  • Participate in quality improvement and postacute care initiatives
  • Build strong relationships with rural facilities and care teams

 

Qualifications

Required

  • MD or DO
  • Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Active CO medical license (or ability to obtain)
  • DEA license

Preferred

  • Experience in LTC, SNF, geriatrics, postacute care, or rural medicine
  • Familiarity with Medicare and valuebased care
  • Interest in leadership or future program growth
