We’re hiring a Patient Care Coordinator – a dedicated and organized professional who ensures seamless coordination of patient care, supports clinical teams, and serves as a vital link between patients, families, and hospice leadership. This role is essential to maintaining quality care and compliance while fostering compassionate communication.

What You’ll Be Doing
- Maintain clinical responsibility for your team and act as liaison between the Director of Patient Care Services (DPCS), clinical associates, patients, families, and the community
- Manage clinical associates’ schedules, including holidays, vacations, weekends, and daily assignments
- Investigate issues, notify DPCS for corrective action, and review hospice documentation for accuracy
- Participate in implementing clinical policies and procedures aligned with Silverado’s mission and goals
- Ensure compliance with local, state, and federal regulations and hospice standards
- Assist in planning and implementing hospice Interdisciplinary Group (IDG) conferences
Qualifications
- Previous healthcare experience in an administrative setting required; hospice experience preferred
- Strong background in home health, healthcare, or hospice preferred
- Minimum of 1 year experience in a clinical healthcare setting (hospital, clinic, or home health agency)
- Reliable transportation to meet attendance and punctuality standards (driver’s license not required unless specified)

$29 - $31 USD
