By acknowledging below, I voluntarily agree to the following: 1. I understand Silverado does not hire everyone who applies. 2. I understand Silverado may not be able to interview me today, and may or may not call me another day for an interview. 3. I understand Silverado does not make hiring decisions instantly. Depending upon the number of applicants, decisions may take several days or longer. 4. I understand hiring decisions are based on a number of factors. I understand that, if hired, I will be an at-will employee, which means that I may terminate my employment at any time, that Silverado may transfer, reassign, suspend or demote me at any time, and that my employment may be terminated at any time, with or without notice and with or without cause. 10. I understand that no one has any authority to enter into any agreement of employment for any specified period of time, or to make any agreement contrary to the foregoing other than in a writing signed by me and the President of Silverado. 11. I understand and agree to allow Silverado to verify the information provided. I further understand that any falsification of information will constitute grounds for immediate dismissal upon discovery thereof. 12. I give Silverado permission to contact any or all of my previous employers and references and developed references for full information and hereby release Silverado from any and all liability for doing so. 13. I understand that any offer of employment is contingent on passing a background check and post-offer, pre-employment drug test and physical (including a background check/clearance with state-specific licensing, misconduct and/or nurse aide registries, and education as required). I have read, understand and agree to comply with these policies. I affirm that the information I provide about myself on application forms, surveys, tests, and during interviews is true and correct. I understand the information I provide will be used in making hiring decisions. I consent to it being used for this purpose and I hereby waive any claims that I have, or might have, regarding the use of this information for hiring decisions.
