Staff AI Engineer

San Francisco
At Sirona Medical we’re building tools for physicians to work as fast as they can think.  Many billions of patient images are acquired each year in the U.S., and nearly all of them are reviewed and diagnosed by a radiologist. In fact, 80% of healthcare data flows through radiology IT systems, and radiologists are among healthcare’s most tech-savvy and influential physicians. But they desperately need better, user-friendly software that cuts clicks and optimizes diagnostic time. Sirona is building a modern cloud-native and AI-powered operating system (“RadOS”) that unifies radiology IT onto a single, streamlined workspace. We’re a San Francisco, CA-based software company (with employees working from all over the world) founded on a deep understanding of both the practice and business of radiology.

About the Role

We are seeking a Staff AI Engineer to architect, develop, and lead the implementation of groundbreaking AI-driven capabilities at Sirona Medical. In this pivotal role, you’ll work at the forefront of applied AI—leveraging Large Language Models (LLMs), multimodal AI, and advanced agentic workflows to build real-world healthcare solutions that improve clinical outcomes and operational efficiency.

You’ll collaborate closely with a multidisciplinary team of engineers, AI researchers, product designers, and radiologists to deliver high-impact features, scalable APIs, and next-generation AI-integrated tooling. Ideally, you are comfortable working across the stack, with a preference toward backend systems and infrastructure, but with experience or interest in frontend development. As a senior member of the team, you’ll help define our technical architecture, shape the roadmap, mentor engineers, and lead the application of cutting-edge AI in one of the most consequential areas of medicine.

Key Responsibilities

  • Develop and scale internal and external APIs and SDKs that power intelligent features and real-time clinical automation.
  • Architect, prototype, and implement agentic AI workflows using cutting-edge LLM and multimodal AI technologies.
  • Collaborate with product, engineering, and AI research teams to ship production-ready LLM- and CV-powered features in a modern cloud environment.
  • Own and evolve our AI training, evaluation, benchmarking and inference infrastructure, helping the team continuously raise the bar on performance, quality and safety.
  • Support applied research and R&D efforts through internal tooling development.
  • Work cross-functionally with product and domain experts to prototype novel use cases (e.g., imaging workflow automation, clinical summarization, EMR integration).
  • Mentor other engineers and help set technical direction across AI product initiatives.

Key Requirements

  • 8+ years of software engineering experience, with significant time spent building AI/ML-powered systems in production.
  • Extensive experience designing and implementing scalable APIs, with strong experience in SQL and/or NoSQL databases.
  • Deep understanding of MLOps best practices including model serving, observability, versioning, and lifecycle management.
  • Experience designing and implementing agentic workflows, leveraging multi-agent systems, LLM-based orchestration, and tools like LangChain, CrewAI, or AutoGen.
  • Strong understanding of retrieval-augmented generation (RAG), planning-execution loops, and decision-making architectures for task automation.
  • Proficiency in Python and modern backend frameworks (e.g., Django, FastAPI).
  • Strong understanding of data structures, distributed systems, and asynchronous processing (e.g., Celery).
  • Strong written and verbal communication skills, including experience writing design documents, RFCs, and collaborating across disciplines.

Preferred

  • Prior experience in healthtech, especially with DICOM, HL7, or EMR/EHR data.
  • Front-end development experience, especially in React and GraphQL, and ability to contribute meaningfully to UI/UX workflows.
  • Open source contributions, public talks, or publications in the AI/ML systems space.

Why Join Us?

  • Make a Real Impact: Your work will directly affect patient outcomes and improve clinical workflows for healthcare providers.
  • Work with Cutting-Edge AI: You'll be at the forefront of applying AI innovations in a high-stakes domain.
  • Collaborate with Top Talent: Join a passionate, multidisciplinary team of AI experts, engineers, and clinicians.
  • Opportunity to Grow: We're a startup—there's room for you to grow and lead as we continue to scale
