Nurse Practitioner / Physician Assistant - Hospital Consult Float

Role Summary 

As a Steadfast Health Float APP, you’ll practice at the top of your license, providing compassionate, evidence-based addiction medicine care across our inpatient consult and outpatient programs. Your primary focus will be supporting Tennessee inpatient consult overflow and emergent outpatient inductions, helping ensure patients receive timely treatment recommendations, medication initiation when clinically appropriate, and seamless connection to ongoing care.

You'll use Motivational Interviewing principles to meet people where they are, often at a pivotal moment in their hospitalization.

This is a virtual-first role, with preference given to candidates based in the Nashville area to preserve the flexibility to provide in-person hospital support when needed. You’ll work closely with hospital providers, nursing teams, case managers, outpatient teams, and Steadfast leadership, with physician backup readily available.

Responsibilities 

  • Provide coverage for Tennessee inpatient consult overflow, including assessment and management of patients with substance use disorders and related comorbidities. 
  • Conduct emergent outpatient inductions referred through the inpatient consult program, including clinical evaluation, treatment planning, medication initiation when appropriate, patient education, and coordination of follow-up care. 
  • Collaborate closely with hospital teams including attending physicians, hospitalists, nursing staff, and case managers, to develop and implement evidence-based treatment plans. 
  • Provide compassionate, patient-centered advocacy through the application of Motivational Interviewing principles. 
  • Deliver up-to-date, life-saving, cutting-edge substance use disorder therapies with ongoing mentorship from specialty-trained addiction experts. 
  • Ensure smooth handoffs to Steadfast Health outpatient clinics and community resources, promoting continuity and reducing barriers to long-term recovery. 
  • Build strong, respectful relationships with hospital and outpatient teams, contributing to an integrated, collaborative model of care. 
  • Remain available for in-person support at Nashville-area partner hospitals when operational or patient needs require it. 
  • Maintain readiness to support Steadfast programs in Ohio or North Carolina if Tennessee demand does not fully utilize the role. 
  • Demonstrate flexibility and proactive problem-solving across changing clinical priorities, coverage needs, and care settings. 
  • Continuously pursue learning opportunities to stay current on addiction medicine and inpatient, outpatient, and virtual-care best practices. 

Qualifications 

  • NP (FNP, AGNP, PMHNP preferred) or PA-C (Internal, Family, Emergency, or Hospital Medicine experience preferred).  
  • Licensed in the state of Tennessee, with willingness and ability to obtain and maintain licensure in Ohio and North Carolina, with the financial and administrative support of Steadfast Health.
  • Minimum of 1–2 years of hospital-based clinical experience strongly preferred. 
  • Comprehensive medical and/or psychiatric evaluation and management experience in substance use disorder treatment. 
  • Knowledge of substance use disorder diagnoses, medications, induction protocols, and treatment modalities. 
  • Ability to confidently navigate hospital systems and technology, outpatient workflows, virtual-care platforms, and interdisciplinary teams. 
  • Excellent interpersonal and communication skills, with the ability to build trust with patients and collaborate effectively with hospital and outpatient staff. 
  • Comfort working independently in a virtual environment while maintaining close communication with physician backup, operational leaders, and care teams. 
  • Nashville-area location preferred to enable in-person hospital coverage when needed.

  • Ability to perform the essential duties of a virtual clinical role, including extended periods using a computer, video, telephone, and electronic health record systems. 
  • Manual dexterity and fine motor skills to operate computer and medical equipment, perform examinations when working in person, and complete electronic documentation. 
  • Ability to communicate clearly with patients, families, hospital staff, and Steadfast team members, both verbally and in writing. 
  • For occasional in-person hospital coverage, ability to move throughout hospital units, including frequent walking and standing. 
  • Capacity to occasionally lift or move items up to 25 pounds, such as medical supplies. 
  • Visual and auditory acuity sufficient to assess patient conditions, review medical records, conduct virtual visits, and respond to clinical situations. 
  • Flexibility to adapt to fast-paced, variable inpatient, outpatient, and virtual-care environments while maintaining a patient-centered focus. 

Schedule: This role follows the anticipated schedule of Monday - Friday 8:30a-5p. Coverage needs may vary based on Tennessee inpatient consult volume, emergent outpatient inductions, and organizational priorities. Please note that the schedule and assigned programs may evolve based on organizational needs, and flexibility may be required as Steadfast Health continues to grow. 

Salary: This is a full-time, W2 position offering a salary of $132,500 - $145,000, commensurate with experience - as well as a 5% monthly performance based bonus
