Advanced Practice Provider - Inpatient SUD Consults

Role Summary
As a Steadfast Health APP, working in our hospital partner settings, you’ll practice at the top of your license, providing a compassionate, evidence-based, addiction medicine consult service, filling a critical gap in substance use disorder care. You will serve as a primary point of contact for providers in the hospital, providing timely consultations, treatment recommendations, and mission-driven support to patients.
As a key member of the hospital-based consult team, you’ll work closely with attending physicians, hospitalists, nursing staff, and case managers. Physician backup will be readily available, and you’ll remain closely connected to Steadfast’s leadership team. Responsibilities
Conduct inpatient consultations for patients with substance use disorders and related comorbidities.
Collaborate closely with hospital teams, including attending physicians, nursing staff, and case managers, to develop and implement evidence-based treatment plans.
Provide compassionate patient-centered advocacy through the application of Motivational Interviewing principles.
Deliver up-to-date, life-saving, cutting-edge SUD therapies with ongoing mentorship from specialty-trained addiction experts.
Assist in ensuring smooth handoffs to outpatient Steadfast Health clinics and community resources, ensuring continuity and reducing barriers to long-term recovery.
Build strong, respectful relationships with hospital staff, contributing to an integrated, collaborative model of care.
Demonstrate flexibility and proactive problem-solving in a dynamic, fast-paced hospital environment.
Continuously pursue learning opportunities to stay current on addiction medicine and hospital-based best practices.
Provide occasional cross-coverage for outpatient clinic APPs as needed, ensuring seamless patient care and team support.
Qualifications
NP (FNP, AGNP, PMHNP preferred) or PA-C (Internal, Family, Emergency, or Hospital Medicine experience preferred), licensed in the state of Tennessee.
Minimum of 1–2 years of hospital-based clinical experience strongly preferred.
Comprehensive medical and/or psychiatric evaluation and management experience in substance use disorder treatment.
Knowledge of SUD diagnoses, medications, and treatment modalities.
Ability to confidently navigate hospital systems, workflows, and interdisciplinary teams.
Excellent interpersonal skills with the ability to build trust with patients and collaborate effectively with hospital staff.

Physical Requirements
Ability to move throughout hospital units, including frequent walking and standing.
Manual dexterity and fine motor skills to operate medical equipment, perform examinations, and complete electronic documentation.

Capacity to occasionally lift or move items up to 25 pounds (such as medical supplies).
Visual and auditory acuity sufficient to assess patient conditions, read medical records, and respond to clinical situations.

Key Responsibilities (include but are not limited to):
Provide individual and group therapy sessions for patients with substance use disorders, both in-person and via telehealth, embodying the Steadfast patient-first ethos.
Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans, modeling a non-paternalistic and destigmatizing approach to patient care.
Utilize evidence-based therapeutic modalities, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Dialectical Behavior Therapy (DBT), to support patient recovery.
Collaborate with all clinic-level team members to formulate and execute patient care plans, including Advanced Practice Providers, Medical Assistants, and Peer Recovery Specialists, ensuring integrated treatment approaches.
Support patients in crisis intervention situations and coordinate higher levels of care when necessary.
Maintain thorough and timely documentation in compliance with state, federal, and organizational guidelines.
Pioneer innovative ways of improving access and overall care for disadvantaged, marginalized populations.
Champion the company’s mission by modeling compassion and fostering trust and strong relationships with patients and communities.
Educate patients and families on SUD, co-occurring disorders, and available resources to support sustained recovery and overall health.
Participate in regular case consultations, team meetings, and continuing education to stay current with best practices in SUD treatment.
Qualifications
Clinical mental health licensure (LMSW, LCSW, LPC-MHSP, LPC-MHSP(t), LMFT) in Tennessee.
Minimum of 1 year experience providing therapy to individuals with substance use disorders.
Certification in addiction counseling is a plus.
Experience with telehealth platforms and virtual care delivery is a plus.
Strong knowledge of SUD diagnoses, medications, and treatment modalities.
Ability to effectively interact with diverse staff, patients, and community members.
Excellent interpersonal skills, a flexible, can-do mentality, and a commitment to continuous learning and professional development.

Location: This role follows a hybrid model, with responsibilities carried out both remotely (at home) and on-site (in clinic). The arrangement is designed to provide flexibility while maintaining alignment with organizational needs and team collaboration.
Schedule: Monday - Friday 8a-5p CST with one work from home day
Compensation: $70,000 - $80,000 with a $1,000 sign on bonus
