What You'll Do
The Medical Director, Nephrology position is a pivotal role responsible for leading nephrology-focused clinical initiatives, enabling nephrology partner integration, and spearheading training and education programs. While Strive Medical Directors for Nephrology do not act as the patient’s attributed nephrologist, they are critical in ensuring that care through the patient’s personal nephrologist is optimized and well-coordinated. The ideal candidate will be instrumental in driving Strive Health's mission of delivering exceptional kidney care and ensuring optimal patient outcomes.
The Medical Director, Nephrology will lead nephrology-focused clinical initiatives, enable nephrology partner integration, and spearhead training and education programs across all regions. While not serving as the patient's attributed nephrologist, this role is critical in ensuring care through the patient's personal nephrologist is optimized and well-coordinated. The Medical Director will design and implement clinical programs that impact key metrics including GDMT adherence, volume management, Optimal Starts rates, and utilization metrics with the goal of delivering exceptional kidney care and ensuring optimal patient outcomes for CKD and ESKD populations. This role reports to the Chief Clinical Officer.
- Lead biweekly Optimal Starts rounds with each region to review complex clinical cases, collaboratively problem-solve, and identify provider integration challenges with nephrologists and vascular surgeons while providing patient-level advocacy
- Conduct weekly ESRD rounds with each region to review complex clinical cases in actively dialyzing populations and facilitate multidisciplinary care coordination
- Drive systematic performance improvement through continuous monitoring of key metrics including GDMT adherence, volume management, Optimal Starts rates, readmission rates, and utilization patterns with proactive outlier identification and targeted interventions
- Partner strategically with Regional Medical Directors and Cardiology Medical Director to ensure seamless implementation, oversight, and performance measurement of nephrology-focused clinical programs with robust "cardiorenal" alignment
- Serve as collaborating physician with nurse practitioners through structured biweekly 1:1s and conduct regular 1:1s with RN care managers to provide clinical feedback, enhance kidney care knowledge base, and drive performance optimization
- Develop and disseminate evidence-based educational content on kidney care management best practices while actively participating in comprehensive onboarding of new clinical hires and ongoing training programs
- Maintain and expand direct relationships with new and existing partner nephrologists to foster robust participation and collaboration, including active participation in partner Joint Operating Committee discussions
- Actively serve on central corporate clinical committees and provide expert consultation to product, technology, and operations teams while engaging comprehensively in business development lifecycle from initial client pitches through implementation
- Analyze comprehensive clinical data to identify trends, develop predictive models, create performance dashboards, and implement quality improvement initiatives based on data insights and evidence-based practices
Qualifications
Minimum:
- Medical degree (MD or DO) from accredited institution with Board certification in Nephrology and active medical license
- Graduate of accredited medical school, residency and fellowship programs.
- 6+ years experience of clinical nephrology experience with demonstrated expertise in CKD and ESKD populations across the care continuum
- Current state-specific licensure required.
- Current Drug Enforcement Administration (DEA) license required, or eligible to obtain within 90 days of hire.
- Current BLS or CPR Certification required.
- Board Certified Nephrologist or Internal Medicine provider.
- Efficient and reliable transportation, including an active driver’s license, allowing for the ability to travel across an assigned region to meet patient needs. Locations may include offices, clinics, and patient homes.
- Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting.
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.
Preferred:
- Advanced data analytics capabilities with experience in healthcare performance metrics, predictive modeling, and population health management
- Pre-existing relationships with network providers including nephrologists, primary care physicians, and transplant centers
- Experience with value-based care contracts and understanding of quality metrics, risk adjustment, and financial performance indicators
- Proficiency with clinical technology platforms including EHRs, population health management systems, and performance analytics tools

- Commitment to hands-on clinical engagement with willingness to "get into the weeds" with individual clinicians and cases daily to ensure consistency and quality of interventions
- Exceptional interpersonal and networking skills with proven ability to integrate providers across multiple specialties and build sustainable professional relationships across clinical, operational, and business stakeholders
- Strategic mindset with experience in change management initiatives, cross-functional leadership, and driving adoption of best practices across multiple regions
- Deep understanding of nephrology care pathways, quality measures, regulatory requirements, and emerging trends in nephrology care delivery and value-based payment models
Annual Base Salary Range: $202,000 - $324,000
