Salary Range:

• $25-$30 per hour.

Job Duties:

• Ensure that our clients receive personalized and effective care 
• Conducting comprehensive client assessments and creating initial care plans. 
• Regularly auditing and modifying care plans to align with client needs. 
• Enhancing client care programs and advocating for clients and their families. 
• Maintaining rigorous documentation 
• Ensuring caregivers are fully briefed and aligned with care plans. 

Qualifications:

 

• Has at least two years of experience in a licensed home care organization. 
• Boasts a minimum of two years in a care coordination role within home care or a similar setting. 
• A Bachelor’s degree in a related field is preferred but not required. 
• Is an excellent communicator, detail-oriented, and empathetic.
