Why Elmont?
Elmont, positioned on the border of Queens and Nassau County, is often called "The Gateway to Long Island." It offers a perfect middle ground for those who want the space and community of a suburban neighborhood without sacrificing the accessibility of New York City. Known for its rich horse-racing history and the state-of-the-art UBS Arena, Elmont is a diverse, vibrant community experiencing a significant economic and cultural renaissance.

Residential Environment:
Elmont is a primarily residential community of approximately 36,000 residents. It is characterized by quiet, tree-lined streets featuring a high rate of homeownership. The housing stock consists largely of well-maintained single-family detached homes, including Capes, Colonials, and Ranches, many with spacious yards that are rare closer to the city. It offers a tranquil, "small-town" feel just steps away from the urban energy of the city border.

Transportation:
Elmont is one of the most transit-accessible communities on Long Island. The Elmont-UBS Arena LIRR station provides full-time, year-round service, offering a direct 30-to-35-minute commute to Penn Station and Grand Central Madison. For drivers, the community is bounded by the Cross Island Parkway, providing quick access to the Long Island Expressway (I-495) and the Southern State Parkway. Public bus service via NICE (Nassau Inter-County Express) and MTA Queens buses further connects the area.

Schools:
Education is a cornerstone of the Elmont community. The Elmont Union Free School District is highly regarded, consistently receiving recognition for academic excellence at the elementary level. Older students typically attend schools within the Sewanhaka Central High School District, including the historic Elmont Memorial High School, known for its strong "Model UN" program and competitive athletics.

Shopping and Dining:
Hempstead Turnpike serves as the area’s main commercial artery, lined with diverse dining options ranging from Caribbean and South Asian cuisine to classic American diners. For more extensive shopping, residents are minutes away from the Green Acres Mall in Valley Stream and the high-end retailers at Roosevelt Field Mall in Garden City.

Recreational Activities:
Elmont is home to the legendary Belmont Park, home of the third leg of horse racing’s Triple Crown. The UBS Arena has also transformed the area into a premier destination for NHL hockey and world-class concerts. For outdoor leisure, Hendrickson Park and Valley Stream State Park are nearby, offering trails, playgrounds, and athletic fields.

Cultural and Entertainment Venues:
The cultural heart of Elmont is the energy found around the Belmont Park and UBS Arena district. The Elmont Public Library serves as a vital community hub, offering a theater for local performances, art exhibits, and extensive programming for all ages.

Community Engagement:
Elmont is celebrated for its extreme cultural diversity, with a population representing nations from across the Caribbean, Asia, and Central/South America. This diversity is reflected in the many civic associations, block parties, and the annual "Elmont Day" parade that fosters a strong sense of local pride.

Safety and Security:
The community is served by the Nassau County Police Department (5th Precinct). Elmont maintains a suburban safety profile with active neighborhood watch groups and a strong emphasis on community-oriented policing to support its family-friendly atmosphere.

Overall:
Elmont, NY, offers the space and stability of a suburban lifestyle with the transit links of an urban hub. Its combination of historic landmarks, new multi-billion dollar entertainment investments, and high-performing schools makes it one of the most promising and convenient places to live and work on the Queens-Long Island border.

Patients Come First.
Every decision we make is anchored in strengthening patient care. Our physicians lead with a commitment to quality, access, and outcomes, and our shared infrastructure ensures they have the tools, data, and support to continually elevate care in every community we serve.

A Physician-Led Organization at Every Level.
Our model is built on physician leadership. Through our Physician Executive Committee, Regional Clinical Governance Boards, and Local Head Physicians, clinical decision-making remains with those who deliver care. This physician-led structure, supported by national-scale resources, preserves autonomy while strengthening specialty excellence.

True Partnership That Enables Physicians to Focus on Patients.
Instead of a traditional management model, we operate as a Management Services Organization (MSO). Our national infrastructure, which includes analytics, technology, revenue cycle, compliance, and operational support, reduces administrative burden so physicians can devote their time and expertise to patients. We enable, we support, and we align. We do not manage.

Outcomes Matter.
Our leadership is driven by the measurable value we deliver to physicians, practices, and patients. Our partnership-first approach, combined with data insights and operational strength, has fueled significant clinical, operational, and practice growth. As more top-tier physicians join our specialty-led ecosystem, we continue to expand access and raise the standard of specialty-care nationwide.
