We are seeking a Behavioral Health Consultant (BHC) to join our integrated care team; an ideal opportunity for a licensed therapist who thrives in fast-paced, team-based environments and is passionate about whole-person care in primary or pediatric settings.

Position Overview

The Behavioral Health Consultant (BHC) functions as a core member of the primary care team, providing brief, evidence-based behavioral health interventions within the medical setting.

This role emphasizes:

The BHC helps patients improve both behavioral and physical health outcomes through brief, solution-focused care models.

What You’ll Do

Integrated Patient Care

  • Conduct brief biopsychosocial assessments and diagnostic impressions
  • Deliver same-day, evidence-based interventions (CBT-informed, solution-focused)
  • Perform risk assessments and crisis intervention as needed
  • Provide brief follow-up care and relapse prevention strategies
  • Support management of chronic conditions (e.g., diabetes, pain, insomnia, adherence)

Proactive Identification & Care Coordination

  • Scrub provider schedules to identify behavioral health needs
  • Recommend and facilitate same-day BHC consults
  • Participate in daily huddles, rounding, and interdisciplinary meetings
  • Coordinate with case management, referral partners, and enabling services
  • Triage and connect patients to specialty behavioral health services when needed

Team-Based Collaboration

  • Deliver concise, actionable feedback to medical providers
  • Partner with care teams to improve clinical pathways and outcomes
  • Support integrated treatment planning across disciplines
  • Provide services directly in exam rooms to reduce stigma and increase access

Education & Program Development

  • Deliver psychoeducation tailored to diverse populations
  • Develop and facilitate wellness groups and prevention programs
  • Provide training to medical staff on behavioral health best practices
  • Contribute to clinical protocols and integrated care workflows

Documentation & Compliance

  • Complete timely, accurate EHR documentation
  • Ensure compliance with FQHC, HRSA, state, and federal standards
  • Deliver care that is efficient, culturally responsive, and patient-centered

What You Bring

Required

  • Master’s degree in Social Work, Counseling, Psychology, or related field
  • Indiana licensure: LCSW, LMHC, LMFT (or license-eligible)
  • Experience working with pediatric populations
  • Strong clinical judgment and ability to work in brief intervention models

Preferred

  • Experience in integrated or primary care settings
  • Knowledge of population health and FQHC environments
  • Doctoral degree (PhD or PsyD) from an APA-accredited program
  • Experience with EHR systems and interdisciplinary collaboration
