Licensed Practical Nurse (LPN) - Float
Reports To: Clinical Supervisor / Practice Administrator
Status: Full-Time
We are seeking a Licensed Practical Nurse (LPN)- Float to support our clinical care teams and contribute to high-quality healthcare delivery across our clinics.
Job Summary
The Licensed Practical Nurse (LPN) - Float provides direct patient care and supports the daily clinical operations of JPCHC. The LPN collaborates closely with providers and interdisciplinary care teams to perform patient assessments, administer medications, coordinate lab work, maintain accurate documentation, and help ensure efficient patient flow.
This role plays an important part in advancing JPCHC’s mission to deliver high-quality, patient-centered care while adhering to clinical standards, safety protocols, and regulatory requirements.
Job Responsibilities
(Include but are not limited to:)
- Perform patient assessments, obtain vital signs, and provide clinical interventions within scope of practice
- This position requires floating between JPCHC clinic locations to meet operational and patient care needs.
- Triage patients in person or by phone to determine appropriate care needs and next steps
- Support chronic disease management and population health initiatives through patient outreach, education, and care coordination
- Administer immunizations, medications, and treatments according to provider orders and clinical protocols
- Provide patient education related to medication adherence, chronic disease management, preventive care, and post-visit instructions
- Assist with patient flow management and collaborate with interdisciplinary teams to optimize clinic operations
- Document patient care accurately and timely in the Electronic Health Record (EHR)
- Coordinate follow-up care related to labs, imaging, referrals, and outside services
- Conduct routine screenings and point-of-care testing such as blood glucose, hemoglobin, and urine testing
- Support care gap closure initiatives including outreach for preventive visits, immunizations, and labs
- Participate in care team huddles and daily coordination meetings
- Precept and mentor new clinical support staff as part of onboarding and team development
- Participate in quality improvement initiatives, audits, and clinical training activities
- Adhere to infection control protocols, safety standards, and HIPAA regulations
- Maintain compliance with clinical, regulatory, and organizational policies
- Perform other duties as assigned by the clinical supervisor or practice manager
Qualifications
To perform this role successfully, candidates should demonstrate:
- Strong organizational skills with the ability to manage multiple priorities
- Critical thinking and sound clinical judgment
- Commitment to patient safety, teamwork, and quality outcomes
- Knowledge of the Indiana Nurse Practice Act
- Compassionate, professional, and nonjudgmental patient care approach
- Strong communication and interpersonal skills
- Proficiency with electronic health records and Microsoft Office applications
- Commitment to ongoing professional development and learning
Education & Experience
- High school diploma or equivalent required
- Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Indiana required
- Minimum 1–2 years of clinical nursing experience required
- Experience in primary care, pediatrics, women’s health, behavioral health, or community health strongly preferred
- Basic Life Support (BLS) certification required and must be maintained throughout employment
