About the Role
We are hiring a Primary Care Physician to join our San Francisco team in a unique role that combines direct patient care with protected time to help shape and improve Tia’s clinical model.
This role is designed for a physician who loves caring for patients and is also energized by improving how care is delivered. You will spend approximately three days each week providing relationship-based primary care in our San Francisco clinic and two days partnering with clinical, product, and operational leaders on care model innovation.
During your protected innovation time, you may help design clinical protocols, improve patient care journeys, develop provider tools and workflows, evaluate new services, and translate clinical evidence into scalable care models.
This is not a traditional administrative or medical director position. It is an opportunity for a practicing physician to bring frontline clinical insight directly into the design of a better healthcare experience.

What You’ll Do
Provide exceptional primary care
- Deliver comprehensive outpatient primary care, including preventive care, acute concerns, and chronic condition management
- Build trusted, longitudinal relationships with patients
- Incorporate women’s health needs throughout the primary care experience
- Collaborate with gynecology, mental health, nursing, and wellness colleagues
- Provide evidence-based, inclusive, and patient-centered care
Help improve how care is delivered
- Use protected innovation time to improve Tia’s clinical programs, workflows, and care model
- Design and refine clinical protocols, care pathways, and patient journeys
- Partner with clinical, product, operations, and technology teams
- Identify opportunities to improve quality, access, efficiency, and the provider experience
- Translate clinical guidelines and frontline insights into practical, scalable solutions
- Support pilots and evaluate whether new programs are achieving their intended outcomes
What You’ll Bring
- MD or DO with board certification in Family Medicine or Internal Medicine
- Active California medical license, or ability to obtain one before starting
- Experience providing comprehensive outpatient primary care
- Strong interest in women’s health and whole-person care
- Interest or experience in clinical innovation, quality improvement, care model design, workflow development, population health, or healthcare technology
- Ability to move between direct patient care and cross-functional project work
- Strong clinical judgment, communication, and collaboration skills
- Comfort working in an evolving healthcare environment
You do not need to come from a formal innovation, product, or medical director role. Physicians who have improved workflows, developed protocols, led quality initiatives, built new programs, or identified better ways to deliver care are encouraged to apply.
Work Location
This is a full-time position based in our San Francisco clinic.
The anticipated schedule includes:
- Approximately three days per week providing in-person clinical care
- Approximately two protected days per week focused on care model innovation and cross-functional work

Compensation & Benefits
Tia is committed to pay equity and pay transparency. Compensation for this role will be determined based on job-related factors, including experience, skills, qualifications, location, role leveling, business needs, and market conditions.
The expected salary range for this full-time role is $250,000 to $300,000 annually.
