Vista Del Mar provides a trauma-responsive continuum of services to empower children, youth, and families in Southern California to lead fulfilling lives.

Role Overview:

The Family Facilitator I is responsible for coordinating the activities of the Child and Family Team (CFT) and the development and implementation of individualized CFT plans. Works in collaboration with a Clinical Supervisor to ensure that families receive mental health support, and that Department of Mental Health (DMH) documentation is completed. The position is also responsible for coordinating case management activities.

This opportunity offers monthly productivity incentives up to $500 per month.

Essential Job Duties (other duties may be assigned):

  • Provides leadership in the assessment, development, implementation, and evaluation of individual child and family plans.
  • Coordinates, schedules, and facilitates CFT meetings for development of individualized child and family plans.
  • Coordinates case management activities.
  • Works in collaboration with Clinical Supervisor to ensure completion of DMH paperwork.
  • Works in collaboration with Clinical Supervisor to ensure that child and families receive appropriate mental health support.
  • Secures CFT approval for overall budgeted plans, and monitors Child and Family Plan (CFP) expenses and budget.
  • Monitors and supports the provision of unconditional care by assessing individual plans and making changes as needed, in collaboration with the CFT.
  • Works in collaboration with Clinical Supervisor to manage crises when plans disrupt and provides on-site consultation to others involved with plans.
  • Participates in the 24 hour on-call system.

What You Bring to VDM:

  • BA and/or relevant experience as determined by Program Director—master’s degree preferred.
  • Bilingual Spanish-English is required.
  • 2 or more years of direct practice experience working with seriously disturbed children adolescents, and families.
  • Experience engaging parents as partners and working successfully with families from diverse ethnic and cultural groups.
  • Excellent verbal and written communication skills.
  • Ability to effectively conduct meetings and motivate others to work collaboratively.
  • Strong organizational and planning skills.
  • Excellent person relationship, facilitation, and service management skills.
  • Experience in crisis management is helpful.

Pay Rate: $26.00/hour
