Position Overview:

As an Early Childhood Wellbeing Project Family Resource Specialist, Clinician/Case Manager with FSSM it will be vital to possess and implement the knowledge and educational background applicable to work with individuals, groups, families, other disciplines and community services in the local area. The Family Resource Specialist works with all appropriate systems on behalf of the client. Services will include assertive clinical case management and with the opportunity to carry therapy cases

Essential Job Duties (other duties may be assigned):

When acting in the role of the Early Childhood Wellbeing Project (ECWP) Family Resource Specialist:

  • Assertive clinical case management and linkages to incoming families;
  • Assist in the coordination of service provider network;
  • Refers families and early childcare and education professionals to network service providers;
  • Maintains virtual referral system for network providers;
  • Client advocacy;
  • Outreach on public education with guidance from networks;
  • Tracks project outcomes;
  • Tracks referred families who have gone through our Wellbeing Project referral process;
  • Assists with parent engagement/direct services including parenting classes and therapy as assigned;
  • Will work collaboratively with ECWP Manager on an on-going basis with any or all of the above items.

When acting in a Clinician Role:

  • Complete intake assessments, formulate and implement treatment plans for all clients including any case-management services required;
  • Conduct regular therapy sessions (individual, conjoint, family or group) with each adult, child, parent(s) and significant people, in or out of the office as indicated by the treatment plan and client need;
  • Thoroughly evaluate the progress of each client through regular supervision, treatment plan reviews, outcome measures and file reviews;
  • Consult with Clinical Supervisor on a weekly basis to provide proper clinical care and appropriate interventions;
  • Provide consultation services to teachers and other professionals when requested/necessary;
  • Attend all meetings, groups and training sessions as requested by FSSM Director, Managers, or Coordinators;
  • Maintain client records in accordance with agency policy and standards, including process notes, 90 Day reviews, assessments, outcome measures or other reports;
  • Complete all mandated paperwork (i.e. DMH) as per state requirement;
  • Maintain appropriate billable activity for assigned caseload;
  • Adhere to ethical and legal standards of the agency and profession;
  • Represent the agency in a professional manner in meetings, groups and training sessions;
  • Take responsibility in gathering knowledge and developing skills needed to perform job duties
  • Complete and maintain all training requirements as put forth by program, DMH, and JHACO standards related to their role and above duties. Trainings and certifications include, but may not be limited to the following: Child and Adolescent Needs and Strengths Assessment (CANS), Annual Cultural Competency trainings, Placement Stability DMH training, Fetal Alcohol Syndrome DMH training, Trauma Informed Strategies for Working with Individuals With ID/DD/ASD DMH training, etc.
  • Perform all other tasks as instructed by the ECWP Manager, and/or Directors and consistent with the above duties;

What You Bring to VDM:

  • MSW or MFT, registered as an Associate with the CA BBS (license eligible). License required within 42 months of employment unless stated otherwise in letter of hire;
  • Ability to hear and verbally communicate with others;
  • Must be able to learn/use necessary computer programs;
  • Strong organizational and communication skills.
  • Bilingual, Spanish-speaking, required.

Salary: $70,560 / annually
