The Lead Care Manager provides direct service, as well as coordinates care management and functions as a part of a "Care Team" for the Enhanced Care Management Program (ECM). The Lead Care Manager will be responsible for the oversees of specific cases, coordinate health care benefits, provide education and facilitate client access to care.

Essential Job Duties  

  • Oversees development and implementation of Individualized Care Plan/Health Action Plan.
  • Communicate regularly with other service providers to ensure that care is continuous and integrated among all service Providers and referring to and following up with primary care/physical and developmental health, mental health, SUD treatment, oral health, models trauma-informed care, necessary community-based and social services, Community Supports, and housing, as needed.
  • Providing support to engage the Client in their treatment, including coordination for medication review and/or reconciliation, scheduling appointments, providing appointment reminders, coordinating transportation, accompaniment to critical appointments, and identifying and helping to address other barriers to Client engagement in treatment.
  • Communicating Clients’ needs and preferences timely to all members of the Clients’ care team in a manner that ensures safe, appropriate and effective person-centered care.
  • Regular contact with the Client and their family client(s), guardian, caregiver, and/or authorized support person(s), when appropriate, to help assist with consistency of care plan.
  • Usage of EMR/EHRs/Care Management Software.
  • Work collaboratively with any and all staff regarding Transitional Care Planning.
  • Monitor and ensure effective linkages with community and agency service providers.
  • Participate in training activities as directed.
  • Establish and maintain positive and effective working relationships with internal staff and external resources.
  • Other duties as assigned.

 

What you Bring to VDM

  • BA and/or relevant experience as determined by Supervisor
  • Two or more years of direct practice experience working with seriously emotionally disturbed children, adolescents, and families.
  • Experience in engaging parents/caregivers as partners and working successfully with families from diverse ethnic and cultural groups.
  • Ability to work flexible hours, including occasional night/weekend work.

Salary: $22.00 - $25.00
