POSITION OVERVIEW
Vista Del Mar UAC and Family Services-Vista Safe Homes HSPRS is seeking a dedicated and compassionate Post-Release Service Intensive Level 3 Case Manager (PRS Intensive Level 3 Case Manager) who is a flexible thinker and takes initiative to independently problem-solve in crisis situations. In this crucial role, you will be responsible for providing comprehensive case management services to unaccompanied alien children (UAC) who have been released from federal custody and placed with approved sponsors. The intensive nature of Level 3 post release services reflects the UAC’s higher level of interventions needed to ensure their safety and stabilize the UAC in the home and within their community. Culturally and linguistically appropriate interventions and supportive services are provided to the UAC and the families they are placed with through weekly in-person contacts for the first 45 to 60 days depending on needs(s). Following the end of weekly contacts, monthly contacts will be provided for 6 months from date of acceptance. PRS Intensive Level Case Managers will carry a minimum caseload of (3) Level 3, (3) Level 2 and (3) Level 1 PRS cases.
Essential Job Duties(other duties may be assigned):
- Assessment and Planning: Conduct weekly and then monthly in-person, thorough assessments of each unaccompanied UAC's needs, strengths, and challenges. The in-person assessments are crucial for determining and the UAC’s affect and ability to be a reliable self-reporter. This clinical assessment approach is part of ongoing efforts to build a viable support system, reliable communication conduits and constructive collaboration with the UAC, sponsor, and other household members.
- Crisis Intervention: Be prepared to respond to crises and emergencies affecting UAC, working in collaboration with emergency services and appropriate agencies as necessary. PRS Intensive Level 3 Case Manager will regularly interact with UAC Protection Services, Trafficking Victims Assistance Program and behavioral healthcare providers.
- Case Management: Provide crisis-intervention and ongoing case management services to, including assistance with accessing education, medical care, mental health services, legal assistance, and other essential resources. Conduct UAC centered needs assessments, develops individualized strength-based, goal-oriented service plans, and monitors progress of goal completion. Provides ongoing case management with a focus on screening and making appropriate referrals to address UAC's needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, UAC enrichment activities.
- Advocacy: Serve as an advocate for the rights and well-being of UAC’s, ensuring that they receive fair and appropriate treatment within the community and addressing any issues or concerns promptly. Conduct outreach and education to community service providers to supplement existing services that do not adequately address unique needs of immigrant UAC. Will coordinate referrals and address barriers in relevant service need areas such as legal, education, medical, mental health; supports unaccompanied UAC and sponsor with enrollment and access to services in community.
- Education and Life Skills: Assist UAC in enrolling in educational programs, accessing tutoring, and developing life skills essential for self-sufficiency.
- Cultural Humility: Demonstrate cultural sensitivity and understanding when working with UAC from diverse backgrounds, including immigrants and refugees.
- Documentation: Maintain accurate and up-to-date case records, progress notes, and reports, ensuring compliance with all legal and regulatory requirements.
- Community Partnerships: Collaborate with local organizations, service providers, and community resources to enhance the support network available to UACs.
- Team Collaboration: Work closely with colleagues, including social workers, counselors, and legal professionals, to provide comprehensive care and support to UAC’s.
- Training and Development: Stay informed about changes in immigration policies, laws, and regulations, and participate in relevant training and professional development activities.
- Travel: Frequent local travel such as commuting no more than 120 miles each way to a client’s residence from the PRS Intensive Level 3 Case Manager residence in their personal vehicle is required. The PRS Intensive Level 3 Case Manager will be allowed to rent a vehicle for any local travel that exceeds 120 miles each way. Possible airfare travel is required to support warm handoffs and being assigned a Level 1 or 2 PRS case that is considered non-local.
What you bring to VDM?
- Master’s in social work, psychology, sociology, or other behavioral science in which direct clinical experience is a program requirement, or Bachelor’s and minimum 5 years of clinical experience.
- Must possess knowledge of community social services and working with the refugee populations.
- Licensed or eligible for licensure preferred
- Bilingual in English and Spanish required
- 2 years in case management within UAC welfare programs preferably with immigrant or refugee populations
- A valid California driver’s license, proof of current automobile insurance
Salary - $75,418-$78,418 / annually
