POSITION OVERVIEW

The Intake Coordinator for Vista Safe Homes-Home Studies and Post Release Services (HSPRS) - is responsible for the process of monitoring Post Release Services (PRS) and Home Study (HS) cases throughout the week (including weekends as needed) and completing intakes for all PRS and HS referrals that are accepted through the Unaccompanied Alien Children (UAC) Post Release Services Application (PRS App). The Intake Coordinator works directly with UAC’s, Sponsors, and Office of Refugee Resettlement (ORR) Care Providers to gather and process essential information before assigning a case to a designated Home Study Case Worker/Post Release Services Case Manager (HSPRSCM). Additionally, they will collaborate with HSPRS Supervisors and Quality Assurance personnel to ensure proper documentation and service delivery is in accordance with ORR policy and procedures.

Essential Job Duties (other duties may be assigned):

  • Maintains and enters all intake information into the UAC Portal, PRS App and Salesforce Software (case management system subject to change).
  • Ensures PRS and HS referrals are processed appropriately to guarantee intake criteria have been met.
  • Initiates communication with Sponsors and/or Care Providers by conducting required initial contacts.
  • Classifies, sorts, inputs, and files various records, documents, and materials, including personal identifiable information (PII) and other sensitive data.
  • Monitors updates on policies and procedures that are related to intake, and case file expectations per ORR.
  • Identifies any immediate needs or issues communicated by the UAC, Sponsors, families, and/or Care Providers at intake.
  • Ensures the Initial Reason for Referral (IRR) Form is completed with accuracy and in a timely manner.
  • Communicates with sponsors and UAC in a culturally and ethically appropriate manner.
  • Identifies and reports any issues or concerns with the RS/LCM that may require internal or external transfers of HSPRS referrals.
  • Assigns HS and PRS referrals to designated HSPRSCM’s in alignment with current internal staffing procedures.
  • Facilitates communication with program stakeholders to ensure a smooth transition of care at case assignment while meeting required timelines.
  • Collaborates with administrative staff in conducting ongoing client surveys and conducts data entry as needed. 

What you bring to VDM?

  • High School diploma required. Bachelor’s degree in behavioral sciences, human services, or social service fields preferred
  • Must have a strong knowledge of overall company operations and policies and procedures
  • Must have at least one year’s experience working with youth.
  • Ability to perform work with little to no supervision.
  • Prior experience identifying community social services and other resources.
  • Excellent verbal, written communication skills
  • Proficient in Internet Software – such as databases, hosted systems; Microsoft Office, including Power Point; Excel and other information systems
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations
  • Flexibility in meeting deadlines and adjusting to changing priorities.
  • Ability to troubleshoot issues and partner with all levels of employees to identify effective solutions.
  • Strong time management skills as it related to prioritizing, delegating, overseeing multiple projects, and ability to effectively manage own time.
  • Knowledge of various funders and stakeholders such as The Office of Refugee Resettlement (ORR) and/or The Department of Homeland Security (DHS). (Not required but strongly preferred).
  • Bilingual in English and Spanish (preferred)

Pay Rate: $25.00 / hour
