MISSION:
Vista Del Mar provides a trauma-responsive continuum of services to empower children, youth, and families in Southern California to lead fulfilling lives.
The Lead Medical Coordinator provides advanced non-medical/clerical case coordination and health services support for youth placed in ORR-funded residential shelter care. This role ensures timely access to medical, dental, behavioral health, and specialty services while maintaining compliance with ORR policies, DHUC standards, HIPAA requirements, and agency protocols. The position serves as a central liaison between the Health Center, residential staff, clinicians, external medical providers, and the multidisciplinary treatment team to support youth health outcomes within a trauma-informed, culturally responsive framework. The position requires the ability to work independently, be detailed oriented, exhibit organizational skills, be proactive, and work as a team member.
Essential Job Duties (other duties may be assigned):
- Coordinate and schedule medical, dental, vision, psychiatric, and specialty appointments for UC.
- Verify insurance, track billing and obtain medical treatment authorizations when required. Prepare appointment packets and ensure continuity of care documentation.
- Track follow-up care, lab results, and treatment recommendation
- Maintain health records in alignment with ORR UC Policy Guide and DHUC standards, keep the UC Portal updated, maintain and assist with data entry.
- Maintain current Health Center forms and protocol
- Ensure that all medical charts, HC forms are current and ready for discharge
- Monitor IME timelines, required screenings, and health service deliverables.
- Monitor medication tracking and psychotropic documentation.
- Communicate with the Department of Heath for Unaccompanied Children (DHUC) when ORR medical guidance is needed.
- Communicate health updates to treatment teams
- Serve as liaison and support between nurses, clinicians, case management, residential staff, and external provider to address youth medical and behavioral needs
- Complete missed dose and medical service reports, and health service tracking tools (QA report).
- Maintain supply inventory for Health Center operation
- Collaborate with chart review, monitoring v QA reviews and prepares the medical chart for audit and regulatory requests.
- Always maintain professional relationships with clients and colleagues
- Provide backup administrative support when needed
- Other duties assigned by the Director of Health Service
What you bring to VDM?
- Computer skills sufficient to meet production demands of the job, including Microsoft
- Office, TIER, My Evolve software, and knowledge of the ORR UC Portal.
