MISSION:

Vista Del Mar provides a trauma-responsive continuum of services to empower children, youth, and families in Southern California to lead fulfilling lives.

ROLE OVERVIEW:

Are you passionate about helping youth and young adults navigate life's challenges while making a lasting impact in your community? Vista Del Mar is seeking a compassionate, organized, and motivated Certified Care Coordinator to join our Vista RISE team.

In this rewarding role, you'll serve as a vital connection between participants, families, healthcare providers, and community resources. You'll help remove barriers to treatment, connect individuals with life-changing services, and support long-term recovery through advocacy, collaboration, and person-centered care.

QUALIFICATIONS:

We are looking for someone who has:

  • California certification through CCAPP, CAADE, or CADTP in good standing.
  • At least one (1) year of experience in care coordination, case management, behavioral health, public health, or social services.
  • Experience working with youth and/or young adults, particularly those facing complex life challenges.
  • Knowledge of Substance Use Disorder (SUD) treatment, behavioral health, community resources, and public benefit programs.
  • Excellent communication, organization, and relationship-building skills.
  • The ability to work independently while collaborating with multidisciplinary teams.
  • A passion for equity, cultural humility, and helping individuals succeed.

ESSENTIAL JOB DUTIES include the following. Other duties may be assigned.

  • Coordinate comprehensive care and connect participants with medical, behavioral health, housing, education, employment, and community resources.
  • Develop strong partnerships with community organizations and service providers to ensure timely referrals and continuity of care.
  • Collaborate closely with clinicians to develop and implement individualized treatment and continuing care plans.
  • Assist participants and families with accessing public benefits including Medi-Cal, CalFresh, housing resources, and other supportive services.
  • Advocate for participants by coordinating services with healthcare providers, schools, probation, child welfare agencies, and community partners.
  • Monitor referrals, follow participant progress, and ensure successful engagement with recommended services.
  • Provide support during transitions between levels of care and after discharge to promote long-term recovery and stability.
  • Maintain accurate documentation and care coordination records within the electronic health record system.
  • Participate as an active member of the multidisciplinary treatment team while supporting participant-centered, trauma-informed care.
  • Help educate participants on available resources, treatment options, and self-advocacy skills that promote independence and wellness.

OTHER SKILLS AND ABILITIES:

  • Strong knowledge of community resources and social service systems.
  • Experience with electronic health records (SAGE/PCNX preferred).
  • Familiarity with Drug Medi-Cal, SAPC requirements, and behavioral health systems.
  • Excellent documentation and time management skills.
  • Ability to prioritize multiple cases while maintaining attention to detail.
  • Skilled in motivational interviewing, participant engagement, and collaborative problem solving.
  • Commitment to trauma-informed, culturally responsive, and strengths-based care.

EDUCATION: Certificate or degree in Social Work, Psychology, Public Health, Human Services, or a related field preferred. Training in Motivational Interviewing (MI), Trauma-Informed Care, Harm Reduction, ASAM Continuum, and Cognitive Behavioral Therapy (CBT) is highly desirable.

CERTIFICATES & REQUIREMENTS

  • Current California certification through CCAPP, CAADE, or CADTP.
  • Valid California Driver's License with an acceptable driving record.
  • DOJ, FBI, and Community Care Licensing (CCL) background clearance.
  • TB clearance.
  • First Aid & CPR certification (or ability to obtain within required timeframe).
  • CPI certification (or ability to obtain upon hire).
  • Ability to complete required annual training and continuing education.

Hourly Rate: $24-$30 per hour (or update based on approved compensation range).
