What You’ll Do
Work under the direction of a Licensed Occupational Therapist to deliver personalized, goal-driven care:
- Implement treatment plans to improve patients’ ability to perform daily activities
- Provide hands-on therapy focused on strength, coordination, mobility, and self-care
- Retrain patients in ADLs (dressing, bathing, meal prep, etc.)
- Assess and report on patient progress, function, and home safety needs
- Educate patients and caregivers on techniques, exercises, and safety strategies
- Identify changes in condition and communicate proactively with the OT and care team
- Complete timely, accurate documentation in the EMR
- Support a safe, effective, and coordinated plan of care
What We’re Looking For
We’re seeking COTAs who understand the independence and clinical awareness required in home health:
- Graduate of an accredited Occupational Therapy Assistant program
- Active COTA license/certification in the state of practice
- Home Health experience strongly preferred
- Current CPR certification
- Ability to work independently in a field-based environment
- Strong clinical observation, communication, and time management skills
- Valid driver’s license, reliable transportation, and auto insurance
