RN Case Manager:

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each participant's unique needs

Clinic RN:

  • Conduct comprehensive assessments, administer treatments, and provide health education to participants and their families

Enrollment RN:

  • Conduct thorough assessments to assess eligibility and guide seniors and their families through the enrollment process for our PACE program

Home Health RN:

  • Provide skilled nursing care to seniors, perform home assessments, develop personalized care plans, and coordinate services to meet the evolving needs of our participants

RN Job Requirements:

  • Graduation from an accredited nursing program and a minimum of one (1) year of experience as an RN
  • Current and unencumbered RN license in California
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Prior experience in senior healthcare or PACE preferred, but not required

Salary/Wage base range for this role is $47.74 - $63.02 hourly + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits.  Actual pay will be adjusted based on experience and role.
