Traveling Nurse Provider

The Traveling Nurse Provider works collaboratively with WelbeHealth Physicians and interdisciplinary teams (IDT), providing assessments, planning primary care, and continuing medical education to WelbeHealth participants.
Essential Job Duties:
- Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments, and continuous medical coverage to participants
- Oversee discharge planning from acute or long-term settings
- Maintain participant care in nursing homes, making regular visits as needed
Job Requirements:
- Graduation from an Accredited School of Nursing and a certificate as a Nurse Practitioner or Physician Assistant with Prescriptive Authority
- State license and willing to obtain other state licensures as required
- Active Drug Enforcement Administration (DEA)
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Must be registered with the Home Care Registry
- Minimum of two (2) years of experience working with vulnerable senior populations
- Ability to travel up to 100% of the time, multi-state support depending on business needs (travel expenses covered); reliable means of transportation
- Participate in rotating night and weekend call

Salary/Wage base range for this role is $168,895 - $211,119 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
