Clinical Pharmacist
***Remote with Occasional Travel to WelbeHealth Centers

The Clinical Pharmacist plays a vital role in providing comprehensive pharmaceutical care within the WelbeHealth clinical care model. The Clinical Pharmacist’s primary responsibilities include optimizing medication therapy, ensuring patient safety, maintaining compliance to Medicare and PACE regulations, and improving health outcomes. As a member of Medical Practice, they work closely with the providers on the team to deliver necessary education, trainings, and consultative services on best practices as necessary.
Essential Job Duties:
- Conduct comprehensive medication reviews for participants to identify drug therapy problems, interactions, and opportunities, and maintain accurate and up-to-date medication records, documenting interventions, assessments, and outcomes
- Collaborate with interdisciplinary team (IDT) members including physicians, nurses, and social workers to develop individualized medication management plans
- Provide clinical consultation to healthcare providers regarding drug selection, dosing, administration, and monitoring
- Participate in IDT meetings to discuss participant progress and adjust treatment plans accordingly
- Implement strategies to mitigate medication-related risks including adverse drug reactions, falls, and medication errors
- Participate in quality improvement initiatives aimed at enhancing medication-related outcomes and reducing healthcare costs
- Ensure compliance with regulatory requirements including Medicare Part D regulations, PACE program standards, and state pharmacy regulations
Job Requirements:
- Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program; must have an active and unrestricted license in the state of practice
- Clinical pharmacy residency, fellowship training, and/or board certification preferred, especially in geriatrics or ambulatory care
- Minimum of two (2) years of experience as a clinical pharmacist, preferably in managed care, geriatrics, or ambulatory care
- Experience in managed care, health plans, PBMs, and/or Part D preferred
- Strong clinical knowledge and proficiency in pharmacotherapy, drug interactions, and medication reconciliation
- Must be comfortable with occasional travel to WelbeHealth centers for business needs

Salary/Wage base range for this role is $145,399 - $174,478/ year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
