Transition of Care Coordinator
The WelbeHealth Transition of Care (TOC) Coordinator is responsible for assisting the Transitions of Care team in the provision of safe and efficient delivery of medical services, as directed. Under the general supervision of medical staff, the TOC Coordinator performs essential administrative and operational tasks that facilitate smooth transitions between care settings. This role works collaboratively with members of the Interdisciplinary team. Including, but not limited to, primary care providers, nurse case managers, social work, transportation, and participants to promote the WelbeHealth values, team culture and mission.
II. Duties and Responsibilities:
- Ensure accurate and timely documentation of all TOC-related activities in WelTeam and Athena.
- Create authorization shells for TOC-related services (e.g., transportation, SNF stays).
- Coordinate transportation for participant discharges and transfers to and from skilled nursing facilities (SNFs).
- Arrange delivery of durable medical equipment (DME) and medications upon discharge.
- Monitor designated QuickCap and Athena inboxes and respond appropriately.
- Collaborate closely with the TOC Nurse to support care coordination needs.
- Maintain prompt, clear communication with WelbeHealth Clinic staff to provide TOC updates (including new encounters such as admissions or ER visits) and obtain information necessary to complete assigned tasks.
- Communicate directly with hospital staff as part of coordinating participant care needs.
- Participate in TOC huddles and document relevant updates in Athena.
- Other duties as assigned.
III. Qualifications and Requirements:
- High school diploma or GED required.
- Medical Assistant certification preferred.Experience in healthcare, care coordination, or administrative support preferred.
- Experience with Transitions of Care and related tasks preferred.
